Effects and outcomes of cardiac surgery in patients with cardiometabolic syndrome
ConclusionsPatients with CMS were more likely to present with increased comorbidities. Patients with CMS undergoing CABG were at risk for worse short ‐term secondary postoperative outcomes and reduced long‐term survival. The data supports the need for further investigation for risk reduction surrounding operative revascularization.

Prognostic Factors for Survival After Extracorporeal Membrane Oxygenation for Cardiogenic Shock
The aim of this study is to examine factors that can predict mortality in patients that have veno-arterial extracorporeal membrane oxygenation (VA-ECMO) instituted for cardiogenic shock. A single-center, retrospective study of 127 patients who underwent VA-ECMO for cardiogenic shock between January 2003 and December 2017 was conducted. Eighty-three (65%) patients survived to weaning or bridging therapy. Complications on VA-ECMO include: hemorrhage (40%), stroke (14%), requirement for dialysis (42%), and limb ischemia (24%). Functional outcomes of rehabilitation in patients with subacute stroke on haemodialysis.
CONCLUSION: Dialysis was not an inhibitory factor affecting outcomes during subacute rehabilitation in patients with stroke. Rehabilitation should be considered for patients with subacute stroke requiring haemodialysis, in order to provide every opportunity to achieve adequate functional outcomes. Short- and long-term outcomes in dialysis patients undergoing transcatheter aortic valve implantation: a systematic review and meta-analysis
ConclusionDialysis patients had significantly higher rates of short- and long-term mortality, short-term life threatening and/or major bleeding, permanent pacemaker implantation, and device failure compared to non-dialysis patients. Careful patient selection who would benefit from TAVI in patients with ESRD requiring dialysis, is necessary to prevent high-rates of post-procedural complications.

Heart transplants from donors with hepatitis C may be safe and could help decrease organ shortage
(American Heart Association) One-year survival was 90% for adults with severe heart failure who received a heart transplant from a donor with hepatitis C, which was nearly identical to those who received a heart from donors who did not have hepatitis C (91%). Rates of organ rejection, stroke and kidney dialysis were similar between the two groups. More research is needed to assess longer-term results, however, increased use of hearts from donors with hepatitis C could help overcome the national shortage of donor organs.

Acute kidney injury with neurological features: Beware of the star fruit and its caramboxin
We present a case of a 51-year-old male with paresis and altered mental status. Screening for neurological diseases such as stroke, Guillain-Barre, meningitis and encephalitis were negative. In the evolution, he developed acute kidney failure and was submitted to 4 dialysis sessions. After talking to the family, we discovered he had ingested over 50 star fruits prior to the acute neurologic deficits. He recovered renal function so a renal biopsy was not required. Comparison of risk of stroke in patients treated with peritoneal dialysis and hemodialysis: a systematic review and meta-analysis.

ConclusionOlder ESRD patients with AF who were treated with warfarin had a no difference in stroke risk, lower mortality risk, but increased major bleeding risk. Renal Dysfunction Is Associated With Poststroke Discharge Disposition and In-Hospital Mortality Clinical Sciences
Background and Purpose—Kidney disease is a frequent comorbidity in patients presenting with acute ischemic stroke. We evaluated whether the estimated glomerular filtration rate (eGFR) on admission is associated with poststroke in-hospital mortality or discharge disposition.Methods—In this cohort study, data from ischemic stroke patients in Get With The Guidelines–Stroke linked to fee-for-service Medicare data were analyzed. Association Between Serum Phosphate Levels and Stroke Risk in Patients Undergoing Hemodialysis: The Q-Cohort Study Clinical Sciences
Conclusions— Higher serum phosphate levels were associated with an increased risk of brain hemorrhage, whereas low levels were associated with an increased risk of brain infarction in hemodialysis patients. ConclusionsThe incidence of CIN is low when MMCT is used routinely to assess acute stroke patients. In this population, CIN was a biochemical phenomenon that did not have clinical manifestations, cause chronic kidney disease, require dialysis, or negatively impact on 90‐day mRS outcomes. Renal profiling and waiting for a baseline serum‐creatinine are an unnecessary delay to emergency reperfusion treatment.

Conclusions— Incidence of stroke is high in patients with ESRD on hemodialysis with high case-fatality. Incident hemodialysis patients had the highest stroke incidence. Many, but not all, important risk factors commonly associated with stroke in the general population were not associated with stroke in patients receiving hemodialysis.
