Child-Pugh Class B or C Liver Disease Increases the Risk of Early Mortality in Patients With Hepatitis C Undergoing Elective Total Joint Arthroplasty Regardless of Treatment Status
CONCLUSION: Patients with HCV and a Child-Pugh Class B or C at the time of elective TJA had substantially increased odds of death, regardless of liver function, cirrhosis, age, Model for End-Stage Liver Disease level, HCV treatment, and viral load status. This is similar to the risk of early mortality observed in patients with chronic liver disease undergoing abdominal and cardiac surgery. Surgeons should avoid these major elective procedures in patients with Child-Pugh Class B or C whenever possible. For patients who feel their arthritic symptoms and pain are unbearable, surgeons need to be clear that the risk of death is...

Effects of a wearable sensor –based virtual reality game on upper-extremity function in patients with stroke
Interventions based on the PABLO virtual reality system improved upper extremity hand function, shoulder and elbow movements, and elicited a higher degree of enjoyment from study participants, than did traditional treatment.Trials registration: The study protocol was registered at Clinical Trials.gov PRS (No.NCT04296032).

Waiting more than 24 hours for hip fracture surgery is associated with increased risk of adverse outcomes for sicker patients: a nationwide cohort study of 63,998 patients using the Swedish Hip Fracture Register
CONCLUSION: The associations between waiting > 24 hours for hip fracture surgery and AF, CHF, and acute ischemia suggest that shorter waiting time may reduce adverse outcomes for the sicker patients.PMID:36847752 | PMC:PMC9972166 | DOI:10.2340/17453674.2023.9595

Missed blunt cerebrovascular injuries using current screening criteria — The time for liberalized screening is now
Blunt cerebrovascular injury (BCVI) has been reported in 1-3% of traumatically injured patients [1] and upwards of 9% of patients with severe head injuries [2]. Although rare, this injury has high morbidity and mortality, mainly due to ischemic stroke, if left untreated. Mortality rates between 20-30% in trauma patients with BCVI have been reported [1] and many survivors sustain permanent neurological deficits. Postulated mechanism of injury include extension, compression, or stretching of vessels within the neck [3].

Effects of Patient Comorbidities and Demographics on Episode-of-Care Costs Following Total Shoulder Arthroplasty
CONCLUSION: Providers should be aware that certain demographic variables and comorbidities (history of stroke, dementia, chronic pulmonary disease, anemia, heart disease, advanced age, and elevated body mass index) are associated with an increase in total costs following primary shoulder arthroplasty. Further study is required to determine whether bundled payment target costs should be adjusted to better compensate for specific comorbidities.LEVEL OF EVIDENCE: Level IV case series.PMID:36749879 | DOI:10.5435/JAAOS-D-22-00450

Exploration of arm weight effects on hemiparetic stroke participants' gait performance
This study's exploratory results demonstrate arm weight's potential for improving higher-functioning stroke survivors' gait performance. Arm weight addition merits further investigation as a possible rehabilitation intervention in the stroke population.

Comparison of transcutaneous electrical stimulation and suprascapular nerve blockage for the treatment of hemiplegic shoulder pain
CONCLUSION: TENS and SSNB were beneficial in relieving pain and increasing passive shoulder ROM and ADL in all patients. The alleviating of pain was faster in patients who underwent SSNB.PMID:36565102 | DOI:10.3233/BMR-220189
