A case of ischemic stroke associated with protein-losing gastroenteropathy and protein S deficiency
We report a 24-year-old woman who developed acute dysarthria and right hemiplegia 4 months after delivering her first baby by cesarean section. Diffusion-weighted magnetic resonance imaging showed a high-intensity signal in the left anterior cerebral artery territory and middle cerebral artery territory.

Continuous transthoracic echocardiographic monitoring for changes in maternal cardiac hemodynamics during cesarean section under combined epidural –spinal anesthesia: a prospective, observational study
In conclusion, continuous transthoracic echocardiographic monitoring revealed that there were significant changes in hemodynamic parameters during cesarean section after delivery of the newborn and placenta, which warranted furt her investigation.

Tranexamic acid minimized perioperative bleeding in patients undergoing noncardiac surgery
Dr. Menon Clinical question: Does tranexamic acid reduce the incidence of life-threatening perioperative bleeding in patients undergoing noncardiac surgery without increasing the risk of major cardiovascular adverse events? Background: Large surgical trials have shown that tranexamic acid reduces the incidence and severity of perioperative bleeding in patients undergoing a cesarean section or cardiac surgery. Other, smaller trials have suggested similar findings with tranexamic acid in patients undergoing orthopedic surgery as well. But, there are limited data on the use of tranexamic acid in patients undergoing non-ortho...

Effect of Dexmedetomidine on Cardiac Output among Parturient with Severe Preeclampsia after Cesarean Section
In conclusion, dexmedetomidine reduces cardiac output by inhibiting the acceleration of heart rate without sacrificing myocardial contractility and stroke volume.PMID:35465007 | PMC:PMC9023150 | DOI:10.1155/2022/4742350

Neurocritical Care of the Pregnant Patient
AbstractPurpose of reviewTo summarize recent changes in management and emerging therapies for pregnant neurocritical care patients.Recent findingsDiagnostic and treatment options for managing neurologic emergencies in pregnant patients have expanded with both greater understanding of the effects of imaging modalities and medications on pregnancy and application of standard treatments for non-pregnant patients to pregnant populations. Specifically, this includes cerebrovascular diseases (pregnancy-associated ischemic stroke, pregnancy-associated intracerebral hemorrhage, cerebral venous sinus thrombosis), post-maternal card...

Minimally Invasive Hemodynamic Assessment during Obstetric Hysterectomy for Invasive Placentation with Epidural Anesthesia.
Conclusion: Patients who are in their third trimester and who subsequently underwent obstetric hysterectomy under epidural anesthesia had modest surgical hemodynamic variance and reduced cardiac workload requirements toward the end of the surgery. PMID: 33193758 [PubMed]

Observation of hemodynamic parameters using a non-invasive cardiac output monitor system to identify predictive indicators for post-spinal anesthesia hypotension in parturients undergoing cesarean section.
In conclusion, the baseline CI obtained via the bioreactance-based NICOM™ system may serve as a predictor of post-spinal anesthesia hypotension in parturients regardless of patient position. PMID: 33093906 [PubMed]

Evaluation of hemodynamics in healthy term neonates using ultrasonic cardiac output monitor
ConclusionsCompared to previous studies, we expanded measurements to longer time-points and we analyzed the impact of commonly used clinical variables on hemodynamics during transition phase thus making our data clinically applicable in daily routine. We calculate reference values for each population, which can be of clinical relevance for quick bedside evaluation in neonatal intensive care unit.

Bolus norepinephrine and phenylephrine for maternal hypotension during elective cesarean section with spinal anesthesia: a randomized, double-blinded study.
CONCLUSIONS: Compared to equivalent phenylephrine, intermittent bolus norepinephrine provides a greater CO for management of maternal hypotension during elective cesarean section with spinal anesthesia; however, no obvious maternal or neonatal clinical advantages were observed for norepinephrine. Recurrent Stroke-Like Symptoms After Cesarean Section Deliveries in a Female Patient With X-Linked Charcot-Marie-Tooth Type 1
Conclusion: The diagnosis of CMTX1 should be considered, even in female patients, when the disease presents with recurrent stroke-like symptoms and abnormal white matter signals on MRI. The puerperium after delivery may be one of the precipitating factors.

Posterior reversible encephalopathy syndrome with ischemic infarction complicated with intrauterine fetal death: A case report
We report a case of PRES with stroke in an adult with intrauterine fetal death (IUFD). Patient concerns: A 35-year-old Asian woman with twin pregnancy underwent cesarean section at 32 weeks of gestation because of IUFD. She presented with focal seizures and visual field defect 2 days after undergoing cesarean section. Her blood pressure and kidney, liver, and coagulation functions were normal without proteinuria. Diagnosis: PRES was diagnosed based on a series of brain magnetic resonance imaging findings. Ischemic infarction in the right frontal lobe eventually developed to encephalomalacia. Interventions: The pat...

New Mom With Brain Tumor Turns To Boston Hospital For Keyhole Brain Surgery
BOSTON (CBS) — Imagine giving birth to a premature baby and then being told you have a brain tumor. That’s what happened to a woman from Holden. But thanks to a new approach at Brigham and Women’s Hospital, this new mom was able to have brain surgery and quickly return to her newborn son. At 27 weeks pregnant, Bethany Shea was diagnosed with preeclampsia and had an emergency C-section. Then she went blind. “It was a pregnancy complication due to my high blood pressure,” Bethany explained. Bethany regained her vision, but worried she had had a stroke, doctors ordered an MRI. But instead of a stroke, it reveale...

Maternal cardiovascular function and risk of intrapartum fetal compromise in women undergoing induction of labor: a pilot study.
CONCLUSION: In this pilot study, we demonstrated that pre-labour maternal cardiovascular assessment in women undergoing IOL could be useful for assessing the risk of intrapartum fetal compromise necessitating operative delivery. The addition of SV, SVR and SVRI significantly improved the predictive accuracy of the baseline antenatal model. This article is protected by copyright. All rights reserved.
