Post-Surgical Cognitive Outcomes (S62.001)
CONCLUSIONS:Patients who undergo AVR demonstrate persistent cognitive decline characterized by diminished learning on tests of memory and language and related to perioperative clinical stroke. These findings underscore the importance of ischemic neurologic complications of surgery and the need for improved stroke prevention and treatment strategies.Study Supported by: R01HL084375 to T.F.F.Disclosure: Dr. Giovannetti has nothing to disclose. Transapical Aortic Valve Replacement for Severe Aortic Stenosis: Results From the Nonrandomized Continued Access Cohort of the PARTNER Trial.
CONCLUSIONS: Among the 975 patients in the NRCA-TA cohort, rates of major outcomes including death and stroke compared favorably with outcomes of PMA-TA and SAVR patients enrolled in the PARTNER trial. This trend toward improved outcomes may be attributed to improved patient selection, individual centers surmounting the procedural learning curve, and refinements in surgical technique. Transcatheter aortic valve implantation with the self-expanding Portico ™ valve system in an all-comers population: procedural and clinical outcomes.
CONCLUSIONS: The PorticoTM TAVI system was safe to implant and achieved a high device success rate. With learning curve effects, the device achieves lower rates of PVL and pacemaker implantation and provides adequate clinical and hemodynamic outcomes through 1 year. Personalizing Medicine: Considering Preferences and Values
If you're interested in heart disease -- and who over the age of 40 isn't? -- you may have read an excellent series of articles by reporter Gina Kolata recently published in the New York Times. If you haven't seen it, the series includes pieces on blood pressure, stents, heart attack treatment and a new approach to aortic valve replacement. The heart valve article especially caught my eye, as this is a story I've been watching with personal interest: My 90-year-old mother has aortic stenosis for which surgery has been recommended. In fact, surgery was first recommended for my mom at least six years ago. I know that timing ...

SOURCE 3: 1-year outcomes post-transcatheter aortic valve implantation using the latest generation of the balloon-expandable transcatheter heart valve
ConclusionIn SOURCE 3, we observe a low complication rate and mortality at 1  year. Given the low incidence of higher degree paravalvular leakages, this variable did no longer affect outcome. Clinicaltrial.gov number: NCT02698956.

Preventing brain embolism by using a modified isolation technique in transcatheter aortic valve implantation for a patient with shaggy and porcelain aorta
We report a case of TAVI in a shaggy and porcelain aorta and prevention of cerebral embolism by adopting a modified isolation technique without using conventional protection devices.>

The Transaortic Approach for Transcatheter Aortic Valve Replacement Initial Clinical Experience in the United States
ConclusionsTAVR via the TAO approach is technically feasible, seems to be associated with favorable outcomes, and expands the current alternative options for access sites in patients with inoperable aortic stenosis who are ineligible for TF TAVR.

Anaesthetic management of transcatheter aortic valve implantation: results from the Italian CoreValve registry.
CONCLUSIONS: Our study indicates that, in experienced centres which have gone beyond their initial learning curve with TAVI, the use of local anaesthesia in a selected patient population can be associated with good clinical outcomes. Nevertheless, as severe procedural complications are possible, an anaesthesiologist should always be present as part of the team. PMID: 25772903 [PubMed - as supplied by publisher]

Mid-term study of transcatheter aortic valve implantation in an Asian population with severe aortic stenosis: two-year valve academic research consortium-2 outcomes.
CONCLUSION: Despite the unique clinical differences in an Asian population, this registry demonstrated favourable mid-term clinical and safety outcomes in Asians undergoing TAVI. PMID: 27516113 [PubMed - as supplied by publisher]

What Are the Main Acyanotic Congenital Heart Diseases?
Discussion Congenital heart diseases (CHD) are malformations of the heart and great vessels. It occurs in about 5-8/1000 live births. Cyanotic congenital heart disease is often noted perinatally because of cyanosis, respiratory distress and/or poor feeding or other distress type problems. A review can be found here. Acyanotic congenital heart disease (ACHD) can present at birth but often is seen in older children or adults unless the lesions are severe, especially obstructive lesions. Severe lesions may also cause cyanosis and distress type problems in patients also. Shunting lesions cause problems by diverting blood flo...

Sutureless Valve in Bicuspid Aortic Stenosis: Modified Technique and Midterm Outcome
Conclusions The sutureless valve can achieve satisfactory outcomes in BAV without paravalvular leakage and excellent hemodynamics. It may serve as the benchmark for transcatheter aortic valve implantation in this unique population.
