Multiple Peri-Operative Complications are Associated with Reduced Long Term Amputation Free Survival Following Revascularisation for Lower Limb Peripheral Artery Disease: A Population Based Linked Data Study.
CONCLUSION: Multiple complications after surgery for lower limb PAD carried a compounding risk of reduced long term amputation free survival. Patients experiencing at least one complication form a high risk group that requires increased attention to prevent the potential development of further complications. No Evidence of the “Weekend Effect” in the Northern New South Wales Telestroke Network
Conclusions: The weekend effect is not evident in the Northern NSW telestroke network experience, though this study did identify some opportunities for improvement in the delivery of acute stroke therapies.

Rivaroxaban Plus Aspirin Versus Aspirin Alone in Patients with Prior Percutaneous Coronary Intervention (COMPASS-PCI).
Conclusions: DPI compared with aspirin produced consistent reductions in MACE and mortality but with increased major bleeding with or without prior PCI. Among those with prior PCI one- year and beyond, effects on MACE and mortality were consistent irrespective of time since last PCI. Evaluation of the MCAST, a multidisciplinary toolkit to improve mental capacity assessment.
Conclusions: The MCAST is the first toolkit to support the needs of individuals with communication disabilities during mental capacity assessments. It enables assessors to deliver high quality, legally compliant and confident practice.IMPLICATIONS FOR REHABILITATIONMental capacity assessment practice needs to be improved to maximise patient autonomy, safety and well-being.The MCAST is a paper-based toolkit designed to facilitate and improve mental capacity practice in England and Wales. This study suggests the MCAST would be easy and acceptable to use in healthcare settings and could lead to improvements in assessment quali...

Oxygen sensing; a stunningly elegant molecular machinery highjacked in cancer.
Abstract Oxygen is of fundamental importance for most living organisms, and the maintenance of oxygen homeostasis is a key physiological challenge for all large animals. Oxygen deprivation, hypoxia, is a critical component of many human diseases including cancer, heart disease, stroke, vascular disease, and anaemia. The discovery of oxygen sensing provides fundamental knowledge of a stunningly elegant molecular machinery; it also promises development of new therapeutics for serious diseases such as cancer. As a result of their impressive contributions to our understanding of the mechanisms by which cells sense oxy...

Thousands of excess deaths from cardiovascular disease during the COVID-19 pandemic
(University of Leeds) Thousands of excess deaths from cardiovascular disease during the COVID-19 pandemic A major new study has identified 2085 excess deaths in England and Wales due to heart disease and stroke during the peak of the COVID-19 pandemic. On average, that is 17 deaths each day over four months that probably could have been prevented.

Medical complications of surgery: an orthogeriatrician's perspective
Hip fracture is one of the most common admission diagnoses in the UK, accounting for>75,000 hospital admissions per year in England and Wales. Fractured neck of femur can be classified as a ‘frailty’ presentation because of an average age of>80 years, a clear association with ‘pathological falls’, including syncope, and a high number of co-morbidities. The combination of traumatic injury, frailty and surgery presents significant challenges to the managing team and requires a multidisciplinary approach to reduce the risk of perioperative complications, most commonly p ostoperative delirium, sepsis, cardiac complicat...

Prescribing of direct oral anticoagulants and warfarin to older people with atrial fibrillation in UK general practice: a cohort study
Anticoagulation for stroke prevention in atrial fibrillation (AF) has, historically, been under-used in older people. The aim of this study was to investigate prescribing of oral anticoagulants (OACs) for peop...

Long-Term Outcomes for Patients With Acute Coronary Syndrome and Nonvalvular Atrial Fibrillation
Short-term outcomes are worse for patients with acute coronary syndrome (ACS) with a history of nonvalvular atrial fibrillation (NVAF). However, long-term prognosis remains unclear. We linked administrative health databases to identify patients hospitalized with ACS (ST-elevation myocardial infarction [STEMI], non –STEMI [NSTEMI], and unstable angina) between 2008 and 2019 in Alberta, Canada. Patients were stratified according to history of NVAF before hospitalization. The primary outcome was a composite of all-cause mortality, hospitalization for myocardial infarction, or stroke at 3 years.

Temporal patterns, characteristics, and predictors of clinical outcomes in patients undergoing percutaneous coronary intervention for stent thrombosis
CONCLUSIONS: PCI for ST has declined in frequency over a 7-year period, with most procedures performed for early ST. Among the different times of ST onset, only early ST is associated with worse clinical outcomes after PCI. Routine use of intravascular imaging and newer P2Y12 inhibitors could further improve outcomes in this high-risk procedural group.PMID:35599596 | DOI:10.4244/EIJ-D-22-00049

How are adults with capacity-affecting conditions and associated communication difficulties included in ethically sound research? A documentary-based survey of ethical review and recruitment processes under the research provisions of the Mental Capacity Act (2005) for England and Wales
Conclusions People with capacity-affecting conditions and associated communication difficulties continue to be excluded from research, with recruitment efforts largely concentrated around participant-facing documentation. There is a need for a more nuanced approach if such individuals are to be included in ethically sound research.
