Is serum total bilirubin useful to differentiate cardioembolic stroke from other stroke subtypes?
CONCLUSIONS: Serum TB level was independently associated with cardioembolic stroke. The combination of clinical data and serum TB may be a feasible strategy to diagnose cardioembolic stroke in the acute phase. PMID: 25891436 [PubMed - as supplied by publisher]

Quality of Care and Ischemic Stroke Risk After Hospitalization for Transient Ischemic Attack: Findings From Get With The Guidelines-Stroke Original Articles
Conclusions— TIA patients with high estimated ischemic stroke risk are less likely to receive defect-free care than low-risk patients. Standardized risk assessment and delivery of optimal inpatient care are needed to reduce this risk-treatment mismatch.

Acute Kidney Injury in Acute Stroke. A preliminary study in Hispanic population (P5.041)
Conclusions:The prevalence of AKI after acute stroke in our population is higher than currently reported.Arterial hypertension was observed as a main risk factor. Further analysis with a higher number of patients is needed to confirm this observation and determine its real impact in Hispanic stroke patients.Disclosure: Dr. Olguín-Ramírez has nothing to disclose. Long-Term Risk of Ischemic Stroke After the Cox-Maze III Procedure for Atrial Fibrillation
Conclusions This multicenter study showed a low incidence of perioperative and long-term postoperative ischemic stroke/TIA after CM-III. Although general risk of ischemic stroke/TIA was reduced, patients with CHA2DS2-VASc score 2 or greater had a higher risk compared with score 0 or 1. Complete left atrial appendage excision may be an important reason for the low ischemic stroke rate.

Does CHA2DS2-VASc score predict ischaemic stroke in chronic obstructive pulmonary disease?
Chronic obstructive pulmonary disease (COPD) is associated with a high risk of ischaemic stroke.1 CHA2DS2-VASc score is recommended and is widely used for stroke risk stratification in atrial fibrillation (AF) patients,2 but it has been shown to be an independent predictor of thrombo-embolic events in patients without AF, as well.3 In this issue ofEuropace, Huet al. performed a retrospective cohort study in a very big case sample (1492 COPD patients with AF and 50  343 COPD patients without AF), and reported that COPD patients with a higher CHA2DS2-VASc score were more likely to develop ischaemic stroke whether or not AF...

Stroke Severity and Outcomes in Patients With Newly Diagnosed Atrial Fibrillation
Conclusion: Stroke severity and in-hospital outcomes in patients with newly diagnosed AF did not differ from those in patients with known AF after adjustment for clinically relevant factors. The importance of detection of latent AF and subsequent anticoagulation in preventing severe stroke should be further emphasized.

Left Atrial Dilatation and Reduced Left Ventricular Ejection Fraction Are Associated With Cardioembolic Stroke
Conclusions: LA dilatation along with reduced EF is a reliable predictor of Afib and cardioembolic stroke. Further studies are warranted to determine the benefit of anticoagulation for secondary stroke prevention in such patient population.

Cardiac Risk Factors for Stroke: A Comprehensive Mendelian Randomization Study
CONCLUSIONS: Available genetic data do not support substantial effects of cardiac traits on the risk of stroke beyond known clinical risk factors. Our findings highlight the need to carefully control for confounding and other potential biases in studies examining candidate cardiac risk factors for stroke.PMID:34911345 | DOI:10.1161/STROKEAHA.121.036306

Advances in Stroke Prevention in Atrial Fibrillation: Enhanced Risk Stratification Combined With the Newer Oral Anticoagulants
Abstract Patients with atrial fibrillation (AF) have an increased stroke risk compared with those in sinus rhythm, although the absolute risk for individual patients is modulated by the presence of various additional risk factors. Patient selection for oral anticoagulation for stroke prevention is based on risks of stroke and bleeding. Although CHADS2 (congestive heart failure, hypertension, age ≥75 years, diabetes mellitus, stroke or transient ischemic attack) is the most widely used scheme for evaluating stroke risk in patients with AF, several other stroke risk factors are not included; therefore, many patients' strok...

Risks and Benefits of Anticoagulation in Atrial Fibrillation: Insights From the Outcomes Registry for Better Informed Treatment of Atrial Fibrillation (ORBIT-AF) Registry Original Articles
Conclusions— In community-based outpatients with AF, use of OAC was high and driven by not only predominantly stroke but also bleeding risk. Stroke risk significantly affects OAC use among those with low bleeding risk, whereas those with high bleeding risk demonstrate consistently lower use of OAC regardless of stroke risk.

Relationship between left atrial appendage morphology and stroke in patients with atrial fibrillation
Background: Atrial fibrillation (AF) is an important cause of stroke. Given the morbidity and mortality associated with stroke, the risk stratification of patients based on left atrial appendage (LAA) characteristics is of great interest. Objective: To explore the association between LAA morphology and LAA characteristics including the extent of trabeculations, orifice diameter, and length with prevalent stroke in a large cohort of patients with drug refractory AF who underwent AF ablation to develop mechanistic insight regarding the risk of stroke. Methods: An institutional cohort of 1063 patients referred for AF ablation f...

Clinical Outcomes after Intravenous Fibrinolysis in Cryptogenic Strokes with or without Patent Foramen Ovale
Conclusions: Paradoxical strokes had better outcomes after IV fibrinolysis than other ischemic stroke subtypes, but this difference is attributable to younger age and milder stroke severity on presentation.

National registry‐based case–control study: comorbidity and stroke in young adults
ConclusionsIn addition to vascular risk factors, comorbidity (represented by ICD‐10 chapters) was associated with increased risk of stroke in young individuals. The risk of stroke was further increased with the number of diagnosed ICD‐10 chapters.
