Best Practice Recommendations for Stroke Vascular Imaging During Iodinated Contrast Shortage
GE Healthcare© announced on April 19, 2022, that their main factory and distributor of iodinated contrast had experienced a temporary shutdown because of COVID-19 outbreak in Shanghai, China. This, along with other supply chain issues, led to a worldwide shortage of iodinated contrast agents, Omnipaque and Visipaque. Our Comprehensive Stroke Center was confronted with the cascading effect of this iodinated contrast material shortage. We took immediate steps to revise our protocols and processes to continue to provide high-quality care to our stroke patients. A multidisciplinary working group comprised of representativ...

The mobile sleep medicine model in neurologic practice: Rationale and application
BackgroundUndiagnosed obstructive sleep apnea (OSA) is prevalent in neurological practice and significantly contributes to morbidity and mortality. OSA is prevalent in US adults and causes poor quality sleep and significant neurocognitive, cardiovascular, and cerebrovascular impairments. Timely treatment of OSA reduces cardio-cerebrovascular risks and improves quality of life. However, most of the US population has limited systematic access to sleep medicine care despite its clinical significance.FocusWe discuss the importance of systematic screening, testing, and best-practice management of OSA and hypoventilation/hypoxem...

Rapid Response to the Acute Iodinated Contrast Shortage During the COVID-19 Pandemic: Single-Institution Experience
Modern health care depends upon the use of radiologic imaging for diagnostic and therapeutic purposes. Iodinated contrast media (ICM) is used in radiology primarily for CT and fluoroscopy-guided procedures. Outside radiology, other services that use ICM include cardiology, vascular surgery, neurology, urology, and gastroenterology. Quaternary care academic medical centers with level I trauma services, comprehensive stroke centers, and ST-segment elevation myocardial infarction services all rely heavily upon a steady supply of ICM to treat patients.

Impact of implementing good care and management practice guidelines in carotid revascularization procedures
ConclusionsImplementing GPMG for carotid revascularization resulted in better clinical results, with decreased neurological complications in patients that underwent angioplasty and endarterectomy.

Effect of the COVID-19 lockdown on disease recognition and utilisation of healthcare services in the older population in Germany: a cross-sectional study
ConclusionThe data suggest that organisational changes were adopted quickly by practice management but also raise concerns about the maintenance of routine care. Prospective studies should evaluate the long-term effects of lockdowns on patient-related outcomes.

Diagnostic accuracy of different clinical screening criteria for blunt cerebrovascular injuries compared with liberal state of the art computed tomography angiography in major trauma
BACKGROUND Blunt cerebrovascular injuries (BCVI) can significantly impact morbidity and mortality if undetected and, therefore, untreated. Two diagnostic concepts are standard practice in major trauma management: Application of clinical screening criteria (CSC) does or does not recommend consecutive computed tomography angiography (CTA) of head and neck. In contrast, liberal CTA usage integrates into diagnostic protocols for suspected major trauma. First, this study's objective is to assess diagnostic accuracy of different CSC for BCVI in a population of patients diagnosed with BCVI after the use of liberal CTA. Second, ...

Evaluation and management of blunt cerebrovascular injury: A practice management guideline from the Eastern Association for the Surgery of Trauma
BACKGROUND Blunt cerebrovascular injuries (BCVIs) are associated with significant morbidity and mortality. This guideline evaluates several aspects of BCVI diagnosis and management including the role of screening protocols, criteria for screening cervical spine injuries, and the use of antithrombotic therapy (ATT) and endovascular stents. METHODS Using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology, a taskforce of the Practice Management Guidelines Committee of the Eastern Association for the Surgery of Trauma performed a systematic review and meta-analysis of currently avail...

Objective Evaluation of CT Time Efficiency in Acute Stroke Response
Timely diagnosis and therapy is essential in stroke response, because time-efficient CT imaging in acute stroke can significantly contribute to preserved neural circuitry and improved patient outcomes [1,2]. Streamlining of acute stroke CT procedures can play an important role in reducing door-to-needle times. In addition to rapid imaging, accessibility of imaging series to interpreting radiologists before examination completion is vital for rapid decision making and patient management. Because different series have different urgency (eg, head without contrast scans should be acquired and interpreted and results communicat...

Quality improvement in neurology: Stroke and Stroke Rehabilitation Quality Measurement Set update
Stroke is the fifth leading cause of death in the United States and the second leading cause of disability worldwide.1,2 "On average, every 40 seconds, someone in the United States has a stroke, and someone dies of one approximately every 4 minutes."3 Each year, nearly 800,000 people experience stroke, with up to 185,000 experiencing recurrent stroke events.3 Strokes occur at any age, and risk increases with age.4 Nearly half of older stroke survivors experience moderate to severe disability.5

Management of blunt cerebrovascular injury (BCVI) in the multisystem injury patient with contraindications to immediate anti-thrombotic therapy
Practice management guidelines for screening and treatment of patients with blunt cerebrovascular injury (BCVI) have been associated with a decreased risk of ischemic stroke. Treatment: of patients with BCVI and multisystem injuries that delays immediate antithrombotic therapy remains controversial. The purpose of this study was to determine the timing of BCVI treatment initiation, the incidence of stroke, and bleeding complications as a result of antithrombotic therapy in patients with isolated BCVI in comparison to those with BCVI complicated by multisystem injuries.

Case Report of a Severe Recurrent Tongue Self-Injury in an Infant With Dystonia
Dystonia is characterized by sustained or intermittent muscle contractions causing abnormal, often repetitive movements, postures, or both that are typically patterned, twisting, and sometimes tremulous. It is often initiated or worsened by voluntary action and associated with overflow muscle activation. In this article we report a case of severe oromandibular dystonia, which is a specific form of dystonia characterized by involuntary, action-induced tonic or clonic spasms of the masticatory, lingual, and pharyngeal musculature. Episodes of repeated tongue biting in a 17-month-old girl caused her to stay in the PICU for 4 ...

Enhancing Workflow Analysis in Acute Stroke Patients Using Radiofrequency Identification and Infrared-based Real-Time Location Systems
In the United States, stroke is the third-leading cause of death in women and the fourth in men [1]. The financial and social burdens are substantial, with annual direct and indirect cost of cerebrovascular disease and stroke in the United States approaching an estimated $312.6 billion [1]. “Time Is Brain” encompasses contemporary philosophies in the management of acute ischemic stroke (AIS) [2]. Approximately 1.9 million neurons die for each second of arterial occlusion [2]. Rapid diagnosis and treatment is integral to preserving brain function, and timely brain imaging is a crit ical component in the evaluation of p...

H-type hypertension is an important risk factor of carotid atherosclerotic plaques.
CONCLUSION: H-type hypertension is independently associated with higher risk of carotid atherosclerotic plaques. H-type hypertension and Hcy should be the major intervention measures to decrease the incidence of carotid atherosclerotic plaques as well as the stroke in Health Practice Management.
