Thromboembolism after WATCHMANTM in a clopidogrel non ‐responder: A case for concern?
Abstract Atrial fibrillation (AF) is associated with an increased risk of stroke and thromboembolism (TE). The WATCHMANTM left atrial appendage (LAA) closure device is indicated to reduce the risk of TE from the LAA in patients with non‐valvular AF. Here, we present a case of a patient with device‐related thrombus who suffered a TE event two months after WATCHMANTM LAA closure and two weeks after switching from aspirin plus warfarin to aspirin plus clopidogrel therapy. Laboratory investigation identified the patient to be hypercoagulable and to be a non‐responder to clopidogrel therapy. We discuss the potential role ...

Clinical and procedural outcomes with the SAPIEN 3 versus the SAPIEN XT prosthetic valves in transcatheter aortic valve replacement: A systematic review and meta ‐analysis
ConclusionsPatients receiving the S3V had lower risk of PVL, MVC, bleeding, mortality, and stroke. PPI was somewhat higher in the S3V group. S3V implantation was faster and used less contrast.

Interventional closure of patent foramen ovale with Nit ‐occlud® device in prevention of recurrent neurologic events—Long‐term results
ConclusionsThe Nit‐Occlud PFO device and its delivery system are safe and provides sufficient closure of PFO in patients who suffered from cryptogenic stroke, TIA or paradoxical peripheral embolism. It is associated with high procedural success and favorable rates of complete closure.

Impact of flow, gradient, and left ventricular function on outcomes after transcatheter aortic valve replacement
ConclusionsIn patients undergoing TAVR, low flow was an independent predictor of 1‐year mortality and HF, and a stronger predictor than either low AVG or LVEF. Patients with low flow and preserved EF had increased mortality and HF at 1‐year, while those with low flow and reduced EF had outcomes similar to patients with normal flow.

Vascular complications after balloon aortic valvuloplasty in recent years: Incidence and comparison of two hemostatic devices
ConclusionsVC rate after BAV is fairly low in experienced centers without major differences between the 2 most used VCD.

Comparative outcomes of transcatheter aortic valve replacement in African American and Caucasian patients with severe aortic stenosis
ConclusionsThere was no significant difference in the utilization rates, in‐hospital outcomes, and cost of TAVR between Caucasians and AA patients in contemporary US practice. Further comparative studies of surgical and TAVR in AAs and other racial minorities are warranted.

Rationale and design of a global registry to evaluate real ‐world clinical outcomes in patients with atrial fibrillation and high risk of stroke treated with left atrial appendage occlusion using the AMPLATZER amulet device—Perspective of available/ongoing registries of catheter‐based LAA occlusion
ConclusionsThere is an important need for prospective real‐world clinical data of catheter‐based LAA occlusion given the increasing application of this intervention. The present global real‐world registry will expand our knowledge on peri‐procedural and long‐term outcome of catheter‐based LAA occlusion using a second generation occlusion device.

Single trans ‐septal access technique for left atrial intracardiac echocardiography to guide left atrial appendage closure
ConclusionsThis initial experience suggests that LAA occlusion with the Amplatzer Amulet device using ICE guidance from the left atrium via a single trans‐septal technique is feasible and safe.

Drug ‐eluting stents versus coronary artery bypass grafting for left‐main coronary artery disease
ConclusionsWhen compared with CABG, DES‐PCI for LMCAD was associated with increases in RRV and the composite of death, MI, and RRV (with/without stroke), despite no differences in mortality, MI, stroke, and the composite of death and MI (with/without stroke).

Comparison of local versus general anesthesia in patients undergoing transcatheter aortic valve replacement: A meta ‐analysis
ConclusionOur meta‐analysis suggests that use of LA for TAVR is associated with a lower 30‐day mortality, shorter procedure time, fluoroscopy time, ICU LOS, hospital length of stay, and reduced need for inotropic support.

Carotid artery stenting in clinical practice depending on patient age
ConclusionsPatients aged ≥75 years had poorer outcomes after CAS compared to younger patients. Importantly, age did not influence the outcome in asymptomatic patients and the risk for stroke beyond the periprocedural period.

Duration of dual antiplatelet therapy following drug ‐eluting stent implantation: A systemic review and meta‐analysis of randomized controlled trials with longer follow up
ConclusionsIn the present analysis of RCTs with longer follow up (2 years or longer), S‐DAPT compared with L‐DAPT, was associated with higher rate of MI and lower rate of major bleeding without any significant difference in the rates of all cause mortality, cardiac mortality, ST, TVR, and stroke.
