Meta-analysis of folic acid efficacy trials in stroke prevention: Insight into effect modifiers
Conclusions: Folic acid supplementation could reduce the stroke risk in regions without folic acid fortification, particularly in trials using a relatively low dosage of folic acid and with low vitamin B12 levels.

Probable REM sleep behavior disorder and risk of stroke: A prospective study
Conclusions: Presence of pRBD was associated with a higher risk of developing stroke, including both ischemic and hemorrhagic types. Future studies with clinically confirmed RBD and a longer follow-up would be appropriate to further investigate this association.

Visual discrimination training improves Humphrey perimetry in chronic cortically induced blindness
Conclusions: In adults with chronic cortical visual impairment, the blind field border appears to have enhanced plastic potential, which can be recruited by gaze-controlled visual discrimination training to expand the visible field. Our findings underscore a critical need for future studies to measure the effects of vision restoration approaches on perimetry in larger cohorts of patients.

Integrity of normal-appearing white matter and functional outcomes after acute ischemic stroke
Conclusions: FA decrease in NAWM contralateral to the acute infarct is associated with worse mRS category at 90 days after stroke. These data suggest that white matter integrity may contribute to functional recovery after stroke.

Fine particulate matter exposure and incidence of stroke: A cohort study in Hong Kong
Conclusion: Long-term PM2.5 exposure was associated with higher risk of incident ischemic stroke, but the association with incident hemorrhagic stroke was less clear.

Physical activity, but not body mass index, predicts less disability before and after stroke
Conclusions: Physical inactivity predicts a higher risk of being dependent both before and after stroke.

Teaching NeuroImages: Acute infarction of the left medial lemniscus masquerading as a peripheral neuropathy
A 58-year-old woman with hypertension and bilateral carpal tunnel syndrome presented with acute paresthesias in her right hand and foot. She had decreased sensation to vibration and pinprick in a right-sided stocking-glove distribution. MRI of the pons confirmed an acute infarction of a paramedian branch of the basilar artery in the left medial lemniscus (figure). In the posterior column pathway, sensory projections from the face, arm, and leg are somatotopically arranged medially to laterally within the medial lemniscus.1,2 Although strokes classically present with numbness, both thalamic and medial lemniscal infarcts can...

Predicting growth of brain aneurysms
In their article "The ELAPSS score for prediction of risk of growth of unruptured intracranial aneurysms," Backes et al.1 developed a risk score for aneurysm growth. They followed 1,507 people who had 1,909 unruptured intracranial aneurysms (some people had more than 1 aneurysm). The patients were from several medical centers around the world: Canada, the Netherlands, Finland, China, and Japan. They were all over 18 years old. Their aneurysms were followed by several types of imaging: some centers used MRI, while others used CT angiography, or calibrated digital subtraction angiography. The local doctor decided how of...

A model for predicting the growth of unruptured intracranial aneurysms: Beyond fortune telling
Because of the tremendous initial and long-term morbidity and mortality incurred by intracranial aneurysm rupture, preventive treatment of unruptured intracranial aneurysms is an appealing strategy to prevent subarachnoid hemorrhage (SAH). However, intervention with surgical or endovascular therapy has risk for considerable procedural complications, especially for large or posterior circulation aneurysms.1,2 Therefore, determining the indications for treatment is crucial to optimizing the management of unruptured aneurysms. Approximately 10% to 20% of unruptured aneurysms will enlarge over the first 10 years of follow-up, ...

Teaching NeuroImages: Acute Parinaud syndrome
An 82-year-old man with atrial fibrillation, nonadherent to rivaroxaban, presented with sudden bilateral ptosis. Examination demonstrated bilateral ptosis (figure 1), pupils midline, fixed midsize, not reactive to light but constricting to accommodation, impaired vertical eye movements, paresis of superior greater than inferior rectus, and upgaze convergence nystagmus. MRI showed infarction of bilateral third nerve nuclei and mesial thalami consistent with an artery of Percheron infarct (figure 2), presumed cardioembolic origin. Artery of Percheron is a single P1 branch that supplies the bilateral paramedian thalami and ro...

Distinct white matter alterations following severe stroke: Longitudinal DTI study in neglect
Conclusions: The present data show how stroke entails global alterations of lesion-spared network architecture over time. Sufficiently large lesions of widely interconnected association cortex induce distinct, large-scale structural reorganization in domain-specific network connections. Besides their relevance to unrecovered domain-specific symptoms, these effects might also explain mechanisms of domain-general deficits in stroke patients, pointing to potential targets for therapeutic intervention.
