Stroke nurse navigator utilization reduces unplanned 30-day readmission in stroke patients treated with thrombolysis
ConclusionThe utilization of a stroke nurse navigator team reduced unplanned 30-day readmissions in stroke patients treated with thrombolysis. Further studies are warranted to determine the extent of the results of stroke patients not treated with thrombolysis and to better understand the relationship between resource utilization during the transition period from discharge and quality outcomes in stroke.

Conclusion
There is an increasing volume of research exploring how best to provide follow-up care to people after minor stroke. Personalised, holistic and theory-informed interdisciplinary follow-up is needed that balances education and support needs with adjustment to life after stroke.

This study attempted to identify the individuals with depression among people with aphasia, to understand the influence of socio-demographic factors on depression, and to understand the relationship PSD has with language impairment and physical disability. We conducted a cross-sectional study with 78 participants recruited from three hospital-based speech and language therapy clinics in Sri Lanka. Identifying Factors Related to Serum Lipids Using Multilevel Quantile Model: Analysis of Nationwide STEPs Survey 2016

Effectiveness of Internet-based health management in patients with dyslipidemia: A four-year longitudinal study
