In-hospital stroke and mortality trends after left ventricular assist device implantation in the United States from 2017 to 2019
CONCLUSIONS: Our study found a significant downtrend in the in-hospital mortality rates among patients with LVAD without substantial changes in stroke rate trends over the study timeframe. As stroke rates remained steady, we hypothesize that improved management along with better control of blood pressure, could have played an important role in survival benefit over the study time frame.PMID:37387231 | DOI:10.1177/03913988231183723

ConclusionA range of indicators for the global burden of stroke attributable to HBMI have been on the rise for the past three decades. Tremendous efforts worldwide should be in place to control and treat stroke attributable to HBMI, especially in regions with high-middle and middle SDIs and among middle-aged and aged populations.

Associations between homocysteine and B vitamins and stroke: a cross-sectional study
ConclusionNon-linear associations of plasma Hcy and B vitamins levels with stroke prevalence were found in American adults. These associations may have an implication that higher plasma Hcy levels should be reduced, and plasma vitamin B6, vitamin B12 and folate levels should be moderately improved in stroke prevention. Future studies are needed to verify the causality of these associations and elucidate the underlying mechanisms.

An Explainable Artificial Intelligence Approach for Discovering Social Determinants of Health and Risk Interactions for Stroke in Patients With Atrial Fibrillation
Stroke remains the primary source of morbidity and mortality associated with atrial fibrillation (AF), despite major advances in prevention. Although effective stroke-prevention strategies are available, optimal implementation of these treatments is limited by (1) rudimentary stroke risk stratification tools (i.e., CHADS2-VA2Sc), and (2) disparities in care and outcomes of AF. Over 150,000 yearly strokes in the United States occur in patients with AF.1 Many of these occur in patients with AF who are misclassified as low-risk or fail to receive appropriate therapies because of healthcare disparities.

Stroke is a leading cause of death and disability in the United States, with approximately 795,000 new strokes occurring annually, including acute ischemic stroke (AIS), intracerebral hemorrhage (ICH), and subarachnoid hemorrhage (SAH).1 The Coronavirus Disease 2019 (COVID-19) pandemic has impacted all levels of care for stroke patients, such as delays in initial presentation, reduction in acute therapies, limitations of in-patient resources, delays or lack of initiation of secondary stroke prevention therapy, and limitations in rehabilitation services after hospital discharge.

Conclusions Bypass routing offers modest travel time benefits for LVO patients and incurs modest penalties for non-LVO patients. Differences are greatest in rural areas. A majority of Americans live in areas for which current guidelines recommend bypass.

ConclusionThe utilization of a stroke nurse navigator team reduced unplanned 30-day readmissions in stroke patients treated with thrombolysis. Further studies are warranted to determine the extent of the results of stroke patients not treated with thrombolysis and to better understand the relationship between resource utilization during the transition period from discharge and quality outcomes in stroke.

Incidence of cardiometabolic outcomes among people living with HIV-1 initiated on integrase strand transfer inhibitor versus non-integrase strand transfer inhibitor antiretroviral therapies: a retrospective analysis of insurance claims in the United States
CONCLUSIONS: Over a short average follow-up period of <2 years, INSTI use among treatment-naïve PLWH was associated with an increased risk of several cardiometabolic outcomes, such as CHF, myocardial infarction and lipid disorders, compared to non-INSTI use.
