Is Crash Loading Acceptable in Carotid Artery Stenting?
ConclusionWith the medical regimen used in this study, urgent CAS with CL APT did not produce more ischemic, thrombotic and hemorrhagic complications than longer loading times. However, careful patient selection might be crucial and adequate loading times should remain the standard of care.

Perfusion Changes in Acute Stroke Treated with Theophylline as an Add-on to Thrombolysis
ConclusionThe results indicate that theophylline does not change the perfusion in potentially salvageable penumbral tissue but only affects the rCBV in the infarct core. In contrast to the penumbra, the infarct core is unlikely to be salvageable, which might explain why theophylline failed in clinical trials.

Thrombus Histology of Basilar Artery Occlusions
ConclusionThe results depict a  differing thrombus composition of basilar thrombi in comparison to anterior circulation clots with an overall higher amount of RBC. This may reflect different pathophysiologic processes between anterior and posterior circulation thrombogenesis, e.g. a larger proportion of appositional thrombus gr owth in the posterior circulation.

ConclusionThe CTA-index is an easy to assess imaging parameter on admission CTA in the acute stroke phase and is associated with angiographic and clinical outcome. Cangrelor and Stenting in Acute Ischemic Stroke
ConclusionCangrelor might be a  safe and effective antiplatelet medication owing to its on/off activity for acute stenting in the setting of acute ischemic stroke. Further investigations through randomized studies with larger samples are necessary.

Optical Coherence Tomography as an Adjunct During Carotid Artery Stenting for Carotid Atherosclerotic Disease
ConclusionOptical coherence tomography image acquisition was found to be safe, effective, and to provide valuable information about plaque morphology and stent-vessel interactions. The role of perioperative anticoagulation after stenting should be re-evaluated given the new findings. A  study comparing CAS with and without OCT with a clinical primary outcome such as ipsilateral stroke could help determine if OCT is an innovative tool to guide stenting and antithrombotic management.

Differentiation of Clot Composition Using Conventional and Dual-Energy Computed Tomography
ConclusionThe fibrin content in blood clots is strongly associated with contrast uptake. As previously shown, the density of the clot formations in native CT scans is dependent on the RBC. Our data show that CT density and relative enhancement of clots are independent determinants of clot composition. Using both variables in the CT workup of acute ischemic stroke has the potential to have a  decisive impact on patient stratification for treatment.

Aspiration thrombectomy in clinical routine interventional stroke treatment
ConclusionWhen used as a  first pass attempt AST is a fast and safe approach for patients suffering from stroke due to ELVO. Nonetheless, early conversion to SRT is needed in most cases and leads to overall excellent procedural results with low complication rates.

Glycoprotein IIb/IIIa Inhibitor Bridging and Subsequent Endovascular Therapy in Vertebrobasilar Occlusion in 120 Patients
Conclusions Our results indicate that combined therapy with IV GPI and subsequent endovascular therapy may be a valid treatment strategy in acute VBO. With this treatment approach, a preserved vigilance before treatment and an embolic occlusion type are associated with no to moderate disability.
