E-033 Optimization of endovascular treatment for patients with acute carotid plaque rupture and free- floating thrombus by use of two overlapping closed-cell stents with different radial force
ConclusionUse of the Carotid Wallstent as the first stent to secure the free-floating thrombus allows save delivery of the Xact stent which then adds a second closed-cell mesh layer as well as the needed radial force to expand the stent construct. This approach may decrease if not eliminate the need for post-stent angioplasty in the acute setting thus decreasing risk of thromboembolic complication.Disclosures A. Kuhn: None. J. Singh: None. S. E-055 Internal carotid artery reconstruction with flow diverting stents in the treatment of acute ischemic stroke: technical considerations and medical management
ConclusionsFDS appear to be a safe and effective tool for reconstruction of a symptomatic dissection in tortuous cervical ICAs. Their flexibility allows conformation to complex anatomies and they can be traversed multiple times by aspiration catheters/stentrievers. We found excellent patency at follow up. The unique challenges posed by utilization of high-profile stents in acute stroke require appropriate technical execution and medical management.Disclosures E. Orru: None. F. Bounni: None. M. Efficacy outcomes and safety measures of intravenous tirofiban or eptifibatide for patients with acute ischemic stroke: a systematic review and meta-analysis of prospective studies
AbstractTo review the literature for randomized control trials (RCTs) and prospective cohort studies investigating the safety and efficacy of tirofiban and eptifibatide in patients with acute ischemic stroke (AIS). PubMed, Embase, and the Cochrane library were searched for available papers published up to September 2021. The efficacy was evaluated based on the 3-month favorable outcome [modified Rankin scale (mRS)  = 0–1], functional outcome (mRS = 0–2), and the last available National Institutes of Health Stroke Scale (NIHSS) score measured in each study. Twelve studies (two RCTs and 10 prospective cohorts) an...

Safety and Efficacy of Pharmaco-invasive Approach Using Streptokinase Compared With Primary Percutaneous Coronary Angiography
Conclusion: Utilizing streptokinase in early re-perfused patients by PI approach (≤3 hrs) seems safe and efficient when PPCI delay (>120 min from symptom onset) is the other option.

Potential applications of venom peptides as anti-thrombotic agents in arterial & deep-vein thrombosis.
CONCLUSION: Ancrod isolated from Malayan pit viper venom can cause reduction in level of blood fibrinogen and has been effectively tried in various ischemic conditions, including stroke. In order to search for novel lead molecules, the emphasis should be on isolation and characterization of pharmacologically active snake venoms proteins affecting blood coagulation and platelet aggregation. In this review an attempt has been made to recapitulates and discuss venoms of different animals and arthropod having anticoagulant peptides for their potential use in therapeutics and diagnostics. Shear ‐sensitive nanocapsule drug release for site‐specific inhibition of occlusive thrombus formation
ConclusionsTargeted delivery of eptifibatide by shear‐sensitive nanocapsules offers site‐specific antiplatelet potential, and may form a basis for developing more potent and safer antiplatelet drugs.

Ischemic Stroke After Treatment of Intraprocedural Thrombosis During Stent-Assisted Coiling and Flow Diversion Brief Report
Conclusions—Current protocols for treatment of intraprocedural thrombosis associated with placement of intra-arterial devices were effective in preventing ischemic stroke in ≈80% of cases. Current smoking was the only independent predictor of ischemic stroke.

E-067 Endovascular Approach in the Repair of the Middle Cerebral Artery Aneurysm Including Those with Wide Neck and Complex in Morphology-A Case Series
ConclusionsEndovascular repair of the MCA aneurysm inclosing those with wide neck and complex are not only safe and feasible, but associated with high long-term functional outcome. Therefore, endovascular options must be offered to patients prior to surgical clipping of a MCA aneurysm. Further study may be warranted.DisclosuresY. Lodi: None. V. Reddy: None.

Abstract 245: Cost Effectiveness and Safety of Abbreviated Eptifibatide Infusion in Veterans Undergoing Percutaneous Coronary Intervention Session Title: Poster Session II
Conclusion: In this real world cohort of patients undergoing PCI, an abbreviated 2 hour eptifibatide infusion after PCI was safe and had a cost savings of greater than 50% per patient, when compared with a standard 18 hour infusion. This approach is more cost-effective and should be considered in patients undergoing PCI who are not pretreated with dual antiplatelet therapy.
