P151/175 Direct aspiration with EMBOVAC - first clinical experience and clot composition (results the "PERFECT study)
ConclusionThis is the first clinical evaluation of EMBOVAC. Direct aspiration with EMBOVAC as first line therapy resulted in high rates of reperfusion and favorable clinical outcomes. Clot composition analysis can aid in better understanding of the stroke etiology and means for effective clot removal.Disclosure of InterestAcandis (OJ, CL, JF); Microvention (GM, CC, JF); Stryker (GM, CC, JF); Medtronic (GM, CC, JF); Surge2surgery (BG); MIVI (CC); Phenox (CL, JF); Penumbra (CL, JF); GSK (AS); German Ministry of Science & Education and Economy & Innovation, EU, Hamburgische Investitions-/Förderbank, Philips, B...

Effect of teleconsultation on the application of thrombolytic therapy in stroke patients in the emergency department
ConclusionsTeleconsultation was a successful and reliable strategy in assessing patients with ischemic stroke and making decisions for IV-tPA. Moreover, patient assessment via teleconsultation was less time consuming. The results of the study are promising for the use of teleconsultation in the future.

P64 NIMBUS geometric clot extractor for tough clots: SPERO study results and clot composition
ConclusionsIn challenging cases with tough clot, first pass of NIMBUS retrieved clot richer in fibrin and lower in RBC content than clot retrieved with the first pass of standard MT devices.ReferencesFennell VS, Setlur Nagesh SV, Meess KM, Gutierrez L, James RH, Springer ME, Siddiqui AH. Postoperative thrombotic effects of tranexamic acid in open heart surgery
ConclusionsIn patients receiving tranexamic acid infusion at 50  mg/kg dose, reexploration rates remained at 4.1% even after major cardiac surgeries. No thrombosis, stroke, or seizure were reported. Our findings support that tranexamic acid is a safe drug which has positive effect on reducing perioperative bleeding.

From a direct oral anticoagulant to warfarin: reasons why patients switch
This study was an analysis of prospectively collected data from a 4-year period surveying a warfarin dose adjustment clinic in a large city centre hospital with the primary objective to identify these reasons. In our clinic with 1791 patients annually under review, 40 patients were identified as having switched from a DOAC to warfarin with the most common reasons for switching being bleeding, re-thrombosis and renal deterioration. Other reasons included medication interactions, side effects, antiphospholipid syndrome, valvular replacement or arterial embolism. Clinical events following warfarin commencement were also recor...
