The association between intraoperative fluid management and perioperative allogenic blood transfusion during adolescent idiopathic scoliosis surgery
CONCLUSION: These results indicate a statistical association between the increase in crystalloid intake and the risk of allogenic blood transfusion during surgical correction of adolescent idiopathic scoliosis. CONCLUSIONS: Transfusion of blood products following cardiopulmonary bypass is associated with postoperative adverse outcomes. Opioid-Free Cardiac Surgery: A Multimodal Pain Management Strategy With a Focus on Bilateral Erector Spinae Plane Block Catheters
CARDIAC SURGERY REQUIRING STERNOTOMY presents unique challenges for perioperative and postoperative analgesia. Intraoperatively, the sympathetic response to surgical stimulation must be modulated carefully to prevent excessive bleeding, maintain the integrity of surgical repairs, and decrease the risk of stroke or iatrogenic aortic injury without compromising myocardial and systemic perfusion. This must be achieved in patients whose underlying abnormal physiology, coupled with the effects of the surgical procedure, often result in tenuous hemodynamics.

Use and safety of aprotinin in routine clinical practice: A European postauthorisation safety study conducted in patients undergoing cardiac surgery
CONCLUSION The data in the NApaR indicated that in this patient population, at high risk of death or blood loss undergoing cardiac surgery, including complex cardiac surgeries other than iCABG, the incidence of adverse events is in line with data from current literature, where aprotinin was not used. Impact of Early, Low-Dose Factor VIIa on Subsequent Transfusions and Length of Stay in Cardiac Surgery
OFF-LABEL USE of recombinant activated factor VII (rFVIIa; NovoSeven, Novo Nordisk, Bagsvaerd, Denmark) for perioperative cardiac surgical bleeding has been shown to reduce bleeding,1-4 blood product administration,1,5,6 and the rate of reoperations.4,6,7 However, there also has been an association with increased mortality,8 thrombosis,9-11 stroke,7,11,12 and renal morbidity.8,9 These inconsistent outcomes may be a function of variations in both timing and dosing of rFVIIa.

Puerperium Stroke and Subsequent Tissue-Type Plasminogen Activator–Induced Hemorrhage: A Case Report
We report a case of ischemic stroke in an apparently healthy 35-year-old gravida 2 para 1 who was treated with tissue-type plasminogen activator (tPA) 9 hours after vaginal delivery that resulted in severe hemorrhage. Limited data suggest use of thrombolytics in pregnancy is safe, but there is a paucity of evidence assessing their use immediately postpartum. We describe successful combination of tPA with endovascular mechanical thrombectomy (EMT) for treatment of postpartum stroke, which was followed by extensive uterine bleeding.
