Warfarin faring better: Vitamin K antagonists beat rivaroxaban and apixaban in the INVICTUS and PROACT Xa trials
J Thromb Haemost. 2023 Jul 8:S1538-7836(23)00523-8. doi: 10.1016/j.jtha.2023.06.036. Online ahead of print.ABSTRACTAlthough guidelines give preference to the direct oral anticoagulants (DOACs) over vitamin K antagonists (VKAs) for stroke prevention in most patients with atrial fibrillation (AF), DOACs are not recommended in those with rheumatic heart disease or mechanical heart valves. The results of the INVICTUS trial (Investigation of Rheumatic AF Treatment Using Vitamin K Antagonists, Rivaroxaban or Aspirin Studies), which compared rivaroxaban with a VKA in patients with rheumatic heart disease associated AF, and the PR...

Should Pleiotropic Effects of Antiplatelet Agents Be Considered for Acute Coronary Syndrome?
When he coined the phrase, “A man is as old as his arteries,” Dr. Williams Osler anticipated the concept of the spectrum cardiovascular disease (CVD) by a century. The populations of many countries are becoming older, which has also led to the number of patients suffering from CVD to increase. Global rates of CVD such as ischemic heart disease (IHD) and stroke have been rising since the 1970s and the leading 2 causes of years of life lost in 2017 may continue to be the same in 2040.1 About 9 million deaths were because of IHD in 2017.

The efficacy of dietary approaches to stop hypertension (DASH) diet on lipid profile: A systematic review and meta-analysis of clinical controlled trials
CONCLUSION: Findings of this meta-analysis showed that following the DASH diet had beneficial effects on serum triglycerides and low-density lipoprotein cholesterol, however, it had no effect on serum total cholesterol and high-density lipoprotein cholesterol levels. Based on these results, the DASH diet can be considered a strategy for the prevention and complementary management of dyslipidemia.PMID:37415367 | DOI:10.2174/0929867331666230706102406

A comparative study of life-years lost attributable to air particulate matter in Asia-Pacific and European countries
This study has three aims related to quantifying life-years lost (LYL) attributable to PM, and further separated into ambient PM and household air pollution (HAP): (1) to investigate LYL by causes of death; (2) to compare LYL between Asia-Pacific (APAC) and Europe; and (3) to assess LYL across different socio-demographic index (SDI) countries. The data used come from the Institute for Health Metrics and Evaluation (IHME) and Health Effects Institute (HEI). Our results show that average LYL due to PM in APAC was greater than in Europe, with some Pacific island countries particularly affected by the exposure to HAP. Three qu...

Polymorphisms of TRIB1 Genes for Coronary Artery Disease and Stroke Risk: A Systematic Review and Meta-analysis
CONCLUSIONS: The rs2954029 A allele was significantly associated with an increased risk of CAD and stroke, according to the present meta-analysis. However, the association of rs17321515 polymorphism with susceptibility to CAD and stroke has not been found in this study.PMID:37414350 | DOI:10.1016/j.gene.2023.147613

Syncope Evaluation: Evidence-Based and Economical
This study eliminated low-risk syncope patients and those with non-syncope transient loss of consciousness, such as seizure and head trauma, using a structured approach in the emergency department (ED), with only high-risk syncope patients being admitted. These high-risk syncope patients made up 28% of the patients included in the study. After admission, a simplified Wells’ pulmonary embolism criteria score was calculated, and a D-dimer was obtained. If either was high, the patient was scanned for PE and 17% were found to be positive, with two-thirds of those being found to have large-vessel pulmonary emboli. The bottom ...
