NOTCH3 C201R variant causes cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) that can be confused with early-onset Alzheimer's disease
NOTCH3 is the causative gene for autosomal dominant cerebral arteriopathy with subcortical infarctions and leukoencephalopathy (CADASIL) which is associated with both stroke and dementia. When CADASIL presents primarily as dementia it can be difficult to distinguish from Alzheimer's disease (AD) at both the clinical and neuropathological levels.

CONCLUSIONS: The simultaneous development of frailty and CI is associated with an increased risk of adverse cardiovascular outcomes including death compared with the development of each syndrome alone. The cysteine-altering p.R544C variant in the NOTCH3 gene is a probable candidate for blood pressure and relevant traits in the Taiwan Biobank
ConclusionNOTCH3 is a strong candidate for a role in stroke, dementia, and CADASIL, which has previously been linked to blood pressure changes. While our preliminary study suggests thatNOTCH3 p.R544C may influence blood pressure, stroke, and dementia in the Taiwan Biobank, replication in a well-powered external sample is required. This study also underlines considerable prospects of detecting novel genetic biomarkers in underrepresented worldwide populations.

Post-stroke cognitive impairment (PSCI) refers to a spectrum of cognitive deterioration following stroke, ranging from mild cognitive impairment to dementia. Cortical plastic alterations associated with post-stroke cognitive impairment are not well documented. It is universally acknowledged as a prevalent and severe clinical syndrome that has an adverse influence on the prognosis of stroke patients [1]. Although PSCI is an umbrella term for a variety of pathological conditions, memory, and cognition impairments are the most troubling for stroke survivors and their carers [2].
