Oral Anticoagulation Use and Left Atrial Appendage Occlusion in LAAOS III
CONCLUSIONS: The benefit of LAA occlusion was consistent whether patients were receiving OACs or not. LAA occlusion provides thromboembolism reduction in patients independent of OAC use.PMID:37732457 | DOI:10.1161/CIRCULATIONAHA.122.060315

A comparative study of thoracoscopic left atrial appendage clipping versus stapled resection
CONCLUSIONS: The clipping group demonstrated a higher rate of complete LAA closure compared to the stapled resection group. Close monitoring of patients with residual LAA stumps is essential. Further research with larger cohorts is needed to elucidate impact of residual LAA stump on thromboembolic events.PMID:37734642 | DOI:10.1016/j.athoracsur.2023.09.010

Prevalence of atrial septal defects and patent foramen ovale in a cohort of sudden cardiac death patients undergoing autopsy
CONCLUSIONS: The combined rate of PFO and ASD in a cohort of 517 patients undergoing autopsy was 7.9 %. None of these patients had experienced a cerebrovascular accident. This rate of PFOs appears lower than earlier reports and raises the possibility that the relative risk of an associated stroke could be higher than previously estimated.PMID:37734655 | DOI:10.1016/j.jjcc.2023.09.006

French National Diagnostic and Care Protocol for antiphospholipid syndrome in adults and children
Rev Med Interne. 2023 Sep 19:S0248-8663(23)00711-7. doi: 10.1016/j.revmed.2023.08.004. Online ahead of print.ABSTRACTAntiphospholipid syndrome (APS) is a chronic autoimmune disease involving vascular thrombosis and/or obstetric morbidity and persistent antibodies to phospholipids or certain phospholipid-associated proteins. It is a rare condition in adults and even rarer in children. The diagnosis of APS can be facilitated by the use of classification criteria based on a combination of clinical and biological features. APS may be rapidly progressive with multiple, often synchronous thromboses, resulting in life-threatening...

A cardiovascular polypill for secondary stroke prevention in a tertiary centre in Ghana (SMAART): a phase 2 randomised clinical trial
Lancet Glob Health. 2023 Oct;11(10):e1619-e1628. doi: 10.1016/S2214-109X(23)00347-9.ABSTRACTBACKGROUND: A cardiovascular polypill containing generic drugs might facilitate sustained implementation of and adherence to evidence-based treatments, especially in resource-limited settings. However, the impact of a cardiovascular polypill in mitigating atherosclerotic risk among stroke survivors has not been assessed. We aimed to compare a polypill regimen with usual care on carotid intima-media thickness (CIMT) regression after ischaemic stroke.METHODS: In SMAART, a phase 2 parallel, open-label, assessor-masked, randomised clini...

Autophagy in Intracerebral Hemorrhage: From Mechanism to Regulation
J Integr Neurosci. 2023 Sep 18;22(5):134. doi: 10.31083/j.jin2205134.ABSTRACTIntracerebral hemorrhage (ICH) is the most lethal type of stroke. Secondary injury from ICH determines the recovery, but there is still a lack of effective treatment. The identification of new therapeutic targets may address the current dilemma. The process of autophagy is mediated through the lysosomal pathway and is used to maintain cell homeostasis. Recent studies have advanced our knowledge of autophagy, and in particular its involvement in cell physiology and pathology. Autophagy involves multiple targets and signaling pathways and occurs in ...

Efficacy and Safety of Repetitive Transcranial Magnetic Stimulation for Poststroke Memory Disorder: A Meta-Analysis and Systematic Review
CONCLUSIONS: rTMS may improve memory function, global cognition, and the ability to perform ADL in patients with PSMD. LF-rTMS and HF-rTMS may have equal efficacy for treatment of PSMD. Future studies should consider extending the follow-up period to explore the safety and long-term efficacy of rTMS for treatment of PSMD and the appropriate choice of placebo for clinical trials of this treatment.PMID:37735134 | DOI:10.31083/j.jin2205131
