The C/L Bashing Thread
I've posted variably in the depths of this forum my disdain for C/L. Applause for Glaucomflecken again. This skit encapsulates the pain of C/L, and when a resident the attendings telling you to just please the primary services and just go do the consult. The time I wasted in residency with consults for "patient cried" etc. This skit just brought flash backs. "patient is schizophrenic" <--stable, no symptoms, regular med compliance, in hospital for surgery or something... Read more (Source: Student Doctor Network)

Schizoaffective Disorder and premorbid functioning
Is recovery to premorbid function (with treatment) incompatible with schizoaffective disorder in the same way it would be with schizophrenia? I'm puzzling over whether I would be able to reject a historical diagnosis of schizoaffective disorder in favor of mood disorder with psychotic features in such a case. Expert witness question about life span prediction and lawsuit valuation
I had a recent call with a plaintiff attorney representing family of mid 20s female with schizophrenia who committed suicide while inpatient. They asked if I would be able to refute defense expert's claims that the patient was high risk and had a significantly reduced life expectancy on the order of less than 5 years due schizophrenia and history of suicide attempts. Plaintiff attorney said this prediction was based on a sophisticated predictive model of people with severe mental illness... Read more (Source: Student Doctor Network)

MSW/MPH dual degree?
Hi all, I am currently a BSW student (just switched my major after deciding I want to pursue an MSW) in my last year. My school offers a dual degree MSW/MPH degree, and I was wondering if this is a beneficial route to take? I want to focus on the clinical side of social work surrounding mental health, and particularly I want to work with severe mental illness like schizophrenia and bipolar in adolescent populations (at least at this moment). My goal is to open private practice, but I am open... Read more (Source: Student Doctor Network)

Capacity consult, chronic delusions, refusing pacemaker
Patient in their 60s with schizophrenia, meds optimized on Invega Sustenna, stable at ALF for years. Has chronic delusions of being monitored, doesn't trust machines. Then he started passing out, admitted to the hospital, found to have frequent runs of asystole and cardiology recommends pacemaker. He doesn't want it (it's a machine, of course). He lacks decisional capacity. High likelihood of death without it. What do you think should happen with someone like this. Other than refusing care... Read more (Source: Student Doctor Network)

Benzos vs benztropine for EPS in the elderly
So I got an outpatient consult recently for med recs on an 65ish year old lady with well-documented late-onset schizophrenia (onset around 52-53) who has been on Invega LAI for years and done very well with it. Patient and daughter are adamant about staying on it, as apparently previous periods without meds were disastrous. She's also been taking benztropine 1 mg BID for EPS (appears to be parkinsonism and akathisia) and clonazepam 1 mg daily for anxiety. When clonazepam was previously... Is the primacy effect ever impaired in the normal population?
The primarcy effect is often impaired in Alzheimer’s disease and I believe in schizophrenia. The primacy effect is meant to be caused by the subject rehearsing the first few list items in a loop, in schizophrenia though the theory is that subjects do not do this. Is this ever the case in the standard population, possibly a person implementing a different strategy? (Source: Student Doctor Network)

Opinions on stopping zyprexa
I have a schizophrenic patient who was on saphris doing ok but not great. He stopped it became very psychotic and was started on Invega inpatient. He was still psychotic after initial 156 mg dosage and it was upped to 234 the next dosage. A week after that dosage (because he was still psychotic) Zyprexa 5 mg was added. He then improved. He’s been on the invega 234 plus 5 of zyprexa for a few months and is doing well. He wants to taper the Zyprexa to be on monotherapy. I’m nervous about... Read more (Source: Student Doctor Network)

I graduated with a Biology degree and now wish to be a Clinical Psychologist?
For a while, I wanted to go to med school or PA school, but over the past year, I decided it wasn't for me. I didn't do research in undergrad, as the only research that ever interested me was psychology-based, and the more experienced psych students often took those. I did an independent study on bipolar/schizophrenia, which I presented at my school's psych conference and wrote a long paper that is not published (though I could try publishing it?). Currently, I am applying for research... Read more (Source: Student Doctor Network)

Therapeutic dilemma in a female 72years old patient with Schizophrenia
I was referred a female schizophrenic 72 years old patient. She does not have any positive symptoms right now. But she has been having mixed depressive/anxiety symptoms with anxiety being the prevalent one - starting with the covid19 pandemic. She is afraid to leave the house (she used to every day) and she is afraid to do anything without the presence of his son, who lives with her (she is scared she might fall - she complains of dizziness, or something bad happens to her... Anorgasmia Treatments?
Looking for some input from the hive mind here. 33yo with schizophrenia. Had him on inpt for about 4 months before he was finally stabilised on Abilify Maintena 400mg and olanzapine 10mg. But he’s had Anorgasmia since being on the meds. Not sure which one did it as he came to me on both. No erectile difficulties, he just can’t finish. I tried bupropion and mirtazapine, no effect. Right now I’ve left him on trazodone 50mg qhs and amantadine 100mg BID (amantadine to counteract D2 blockade... Read more (Source: Student Doctor Network)
