I thought OMM would make me a rockstar at USMLE MSK questions. I was wrong.
It seems like a lot of my friends and I struggle with MSK on USMLE because it feels like the OPP COMAT writers purposely design OMM content to be non-overlapping with USMLE MSK content. Case here: Hemorrhagic Shock from Biopsy Lady with antiphospholipid syndrome (also history of PE and DVT) comes to ED with SOB. No PE/DVT, admitted. Presumptive diagnosis of lupus nephritis made, biopsy recommended. Warfarin held, switched to heparin before procedure, then Lovenox afterward (plan to monitor with anti Xa levels). PBI in N1mi (actual radiation question)
We should have a sub-forum where we ask radiation questions... Have a lady with a RUOQ T1N1mi ER/PR+ breast cancer s/p lumpectomy and SLNB with microscopic disease in the SLN. She has lupus with some mild cutaneous manifestations on plaquenil. She doesn't want mastectomy, and have discussed risks. The cavity with margin and level 1/2 could be treated pretty conveniently instead of WBRT, sparing a ton of tissue. Was wondering if this sounds reasonable in this setting. Question about friends patient encounter
Pathologist here....So long story short. My friend went to see a derm for what she thought was a pimple near her eye. The derm did what I think was a punch biopsy on it because he thought she had lupus. My friend had no other symptoms, no other skin lesions. No labs were ordered before biopsy. Now she has a small wound from the biopsy. The ANA was ordered after the biopsy which was negative. when I heard this I was furious. Was the biopsy clinically indicated? I was thinking several... Do you workup and treat painful diseases (not just the pain)?
This is perhaps more of a nuance question, but do you workup and treat painful diseases? For example, of you suspect RA/Lupus/etc, do you do the bloodwork and manage DMARDs, biologics, etc or simply refer to Rheumatology? If you think the patient has osteoporosis, do you do the bone density/DEXA and give Calcium/Vit D, bisphosphonates, etc? If the patient is having possible side effects from opiates, (which may be related to other medical problems as well) such as low testosterone... Prior Service Navy HPSP and Other Issues

Misnomers in Medicine
I've been trying to make a mental note of everything that I've learned which had a contradictory name, but I feel like I'm only scratching the surface and wanted to consult SDN's collective wisdom. Things like "Obliquus Capitis Inferior" which doesn't attach to the skull like the name implies, or the "Patellar Tendon" which actually connects two bones. CRISPR and Gene Editing Treatment
Anyone else find this new treatment exciting? Hopefully one day in the future gene-editing treatment will be a routine, alternative treatment for those with genomic/chromosomal abnormalities (multiple sclerosis, cancer, lupus, etc). Am I too old to become a doctor?
I am 24, I will be 25 this coming September. Medicine and ballet have always been my top things. I'm alone in the world now after helping my mother battle lupus, but she died last year. I endured a very abusive household that didn't allow for me to study the way I wish I would've been able to, in hindsight, but my dream of becoming a doctor has never left me. I feel terrible because I graduated HS when I was 15 years old, and I could've been finished with medical school by now. I am married... Am I too old to become a doctor? Long time lurker here. Enjoyed reading clinical discussions. I'm quite fresh out of training and would appreciate any feedback or wisdom about a recent case: 50 yo F for elective lumbar laminectomy. PMH of lupus anticoagulant with previous right central retinal vein thrombosis on plavix + coumadin, off both for 5 days for surgery. She's on Plaquenil, CellCept, Prednisone for SLE.
