A machine learning approach to predict mortality due to immune-mediated thrombotic thrombocytopenic purpura
CONCLUSION: The USTMA Mortality Index was acceptable for predicting mortality due to acute iTTP in the USTMA registry, but not sensitive enough to rule out death. Identifying patients at high risk of iTTP-related mortality may help individualize care and ultimately improve iTTP survival outcomes. Further studies are needed to provide external validation. Our model is one of many recent examples where machine learning models may show promise in clinical prediction tools in healthcare.PMID:38651093 | PMC:PMC11033197 | DOI:10.1016/j.rpth.2024.102388 (Source: Thrombosis and Haemostasis)

Multi-phased kinetics and interaction of protein kinase signaling in glycoprotein VI-induced platelet αIIbβ3 integrin activation and degranulation
CONCLUSION: Kinetic analysis of GPVI-initiated multisite protein phosphorylation in human platelets demonstrates multiple phases and interactions of tyrosine and serine/threonine kinases with activation-altering feedforward and feedback loops partly involving PKC. The protein kinase inhibitor effects on multisite protein phosphorylation and functional readouts reveal that the signaling network of Syk, Btk and PKC controls platelet granule exocytosis and αIIbβ3 integrin activation.PMID:38653482 | DOI:10.1055/a-2311-0117 (Source: Thrombosis and Haemostasis)

Management of Abnormal Uterine Bleeding on Anticoagulation: The Patient-Clinician Perspective
J Thromb Haemost. 2024 Apr 18:S1538-7836(24)00216-2. doi: 10.1016/j.jtha.2024.04.002. Online ahead of print.ABSTRACTBleeding is a well-recognized side effect of anticoagulant therapy, which is used to treat venous thromboembolism (VTE) in individuals of all ages, including those of female sex, who commonly experience VTE as a complication of hormonal therapies and/or pregnancy. Heavy menstrual bleeding (HMB) is also extremely common in reproductive aged individuals of female sex. Despite these overlapping situations, relatively little attention has been paid to the impact of anticoagulant-associated HMB on treatment strate...

Evaluation, analysis, and reporting of medication adherence for clinical trials of anticoagulants in children: Guidance from the ISTH SSC Subcommittee on Pediatric and Neonatal Thrombosis and Hemostasis
J Thromb Haemost. 2024 Apr 18:S1538-7836(24)00217-4. doi: 10.1016/j.jtha.2024.04.003. Online ahead of print.ABSTRACTIn response to growing recognition that non-adherence prevents children, adolescents, and young adults from achieving the therapeutic benefits of anticoagulant medication, the ISTH SSC Subcommittee on Pediatric and Neonatal Thrombosis and Hemostasis convened a working party on medication adherence. The primary aim of this manuscript was to synthesize recommendations from the larger adherence science literature to provide guidance regarding the classification, collection, and interpretation of anticoagulation ...

Ticagrelor vs clopidogrel when coadministered with bivalirudin in patients with acute coronary syndrome undergoing percutaneous coronary intervention
CONCLUSION: As an initial treatment strategy, bivalirudin plus ticagrelor could reduce the 12-month risk of ischemic events compared with bivalirudin plus clopidogrel significantly without increasing the bleeding risk in ACS patients undergoing PCI.PMID:38623472 | PMC:PMC11017352 | DOI:10.1016/j.rpth.2024.102375 (Source: Thrombosis and Haemostasis)

Tranexamic acid for management of heavy vaginal bleeding: barriers to access and myths surrounding its use
Res Pract Thromb Haemost. 2024 Mar 19;8(3):102389. doi: 10.1016/j.rpth.2024.102389. eCollection 2024 Mar.ABSTRACTTranexamic acid is safe and effective for the treatment of heavy vaginal bleeding during menstruation and childbirth. It improves the quality of life, facilitates participation in school and work, and reduces the risk of death from postpartum hemorrhage. Despite its well-established benefits, individual- and structural-level barriers preclude its widespread utilization, hindering effective patient care and perpetuating health inequities in women's health. We first describe the evidence for the use of tranexamic ...

Testing for the lupus anticoagulant: the good, the bad, and the ugly
Res Pract Thromb Haemost. 2024 Mar 18;8(3):102385. doi: 10.1016/j.rpth.2024.102385. eCollection 2024 Mar.ABSTRACTLupus anticoagulant (LA) represents 1 of the laboratory criteria for classification of patients as having definite antiphospholipid syndrome (APS). The other 2 laboratory criteria are anticardiolipin antibodies and anti-beta2-glycoprotein I antibodies. At least 1 of these antiphospholipid antibody (aPL) tests need to be positive, with evidence of persistence, together with evidence of at least 1 clinical criterion for APS, before a patient can be classified as having definite APS. LA and other aPL assays are als...

Practical Suggestions for an Optimal Management of Vitamin K Antagonists: Italian Federation of Centers for the Diagnosis of Thrombotic Disorders and the Surveillance of the Antithrombotic Therapies (FCSA) Position Paper
Thromb Haemost. 2024 Apr 16. doi: 10.1055/s-0044-1782688. Online ahead of print.ABSTRACTIn the era of direct oral anticoagulants, vitamin K antagonists retain a clinically relevant role in thrombotic disorders. In Italy, approximately 20% of the patients on anticoagulant therapies receives a VKA, in most cases warfarin. The optimal management of this drug is challenging and cannot disregard its intricate and unpredictable pharmacokinetic properties and patient's thrombotic and bleeding risk. Several clinical issues encountered during warfarin treatment are still unanswered and are tentatively addressed by physicians. In th...
