The Many Sides of New Medical Technology
In this volume of The Journal, Silverstein et al examine the use of the Owlet Smart Sock (OSS) in management of supraventricular tachycardia (SVT) in infants. The smart sock is a pulse oximeter-based technology. The investigators found that use of this technology may result in earlier detection of SVT, which may, in turn, result in better clinical outcomes, such as better preserved left ventricular function and shorter length of hospital stay. On the other hand, use of the Smart Sock was also associated with greater medical resource utilization, including more office visits, telephone calls, and emergency department visits...

Can We Adopt Pediatric Palliative Care that is Indivisible with Equity for all?
In this volume of The Journal, two retrospective cohort studies, both based in Ohio, provide insightful data about the deployment of dedicated pediatric palliative care teams. While home-based visits have been a longtime core principle of pediatric hospice care, their value has not been well defined or justified. Thus, Smith et al analyzed healthcare utilization outcomes in a sample of children who received home visits across two well-established pediatric home-based palliative care and hospice programs over 6 years among 195 children, half of whom had neurologic impairment. A Gentle Introduction to Latent Class Analysis for Researchers in Pediatrics
A recent study in The Journal of Pediatrics from O ’Connor et al reports using latent class analysis (LCA) to discover “complexity” subgroups based on cardiac medical histories of 1,482 participants diagnosed with congenital heart disease (CHD).1 Seven medical history variables (including stroke, cardiac surgeries, and cardiac-related hospital izations) were selected based on indications of CHD complexity. Applying LCA, investigators found four subgroups of complexity: mild, moderate 1, moderate 2, and severe. (Source: The Journal of Pediatrics)

Use of Initial Endotracheal versus Intravenous Epinephrine During Neonatal Cardiopulmonary Resuscitation in the Delivery Room: Review of a National Database
Neonatal resuscitation in the delivery room (DR) remains rare, with the majority of infants successfully transitioning from fetal to neonatal life with no additional resuscitative efforts required beyond the initial steps of warming, stimulation, repositioning and if needed, suctioning.1 Just under 10% require additional resuscitative support, usually in the form of positive pressure ventilation (PPV).2, ³ Less than 1% of newborns receive extensive resuscitation in the form of chest compressions ± epinephrine.⁴ If, after PPV and initial attempted ventilation corrective steps, the heart rate remains (Source: The Journal of Pediatrics)
