Discharge to post –acute care and other predictors of prolonged length of stay during the initial COVID-19 surge: a single site analysis
ConclusionPatients hospitalized with COVID-19 during a US COVID-19 surge had a PLOS and high readmission rate. Lack of insurance, an intensive care unit stay and a decision to discharge to a PAC facility were associated with a PLOS. Efforts to decrease LOS and optimize hospital capacity during COVID-19 surges may benefit from focusing on increasing PAC and HHA capacity and resources. A qualitative study exploring challenges and solutions to negotiating goals of care at the end of life in hospital settings
ConclusionPlanning EOL care demands high-level, compassionately skilful and sensitive care and services, which are in line with the patient ’s and family’s wishes. In practice, however, there are many challenges to this, such as ensuring that patients, families, and HCPs are aware of different expectations regarding future health-care possibilities, and that HCPs are prepared for negotiating GoC to achieve quality and safe EOL care in hospitals. ConclusionPAC3’s multicenter QI project increased data submission timeliness in a large pediatric subspecialty registry. Effects of an educational intervention for rational cardiac enzyme requisitions in critically ill patients: a pre –post intervention study
ConclusionOur pre –post interventional study demonstrated a significant decrease in the concomitance of CK-MB and troponin tests. A rational high-value ordering practice of cardiac biomarkers is possible in critically ill patients and might be suitable for educational interventions. Outcomes among patients admitted for non-ST-segment myocardial infarction in the pre-pandemic and pandemic COVID-19 era: Israel Nationwide study
ConclusionIn contrast to previous reports, admission rates of NSTEMI were similar in this nationwide survey during the COVID-19 era. With longer time from symptoms to admission, but with the same time from hospital admission to reperfusion therapy and with similar in-hospital and 30-day mortality rates. A Delphi consensus study to identify priorities for improving and measuring medication safety for intensive care patients on transfer to a hospital ward
ConclusionsOur findings provide identification of priorities for medication-related intervention components to improve medication safety for intensive care patients transferring to a hospital ward. Prioritization is complemented by the identification and socio-technical categorization of barriers and facilitators to intervention delivery. The identified important medication and patient outcomes to measure will inform the design of a future patient medication safety intervention study. A regional approach to reduce postoperative opioid prescribing in Ontario, Canada
Conclusions: Our approach resulted in a significant reduction in the number of postoperative opioids prescribed across a number of surgical specialties. Our findings indicate that evidence-based strategies derived from a regional collaborative network can be leveraged to promote and sustain quality improvement activities. Dedicated teams to optimize quality and safety of surgery: A systematic review
ConclusionsThe institution of surgical teams who followed communicative and/or technical training appeared to have beneficial effects on several clinical outcome measures. Dedicated teams provide a feasible way of improving healthcare quality and patient safety. A dose –response effect of team training was reported, but also a relapse rate, suggesting that repetitive training is of major concern to high-quality patient care.
