Cystoprostatectomy with Recurrence
I have a patient who underwent cystoprostatectomy for high grade urothelial ca throughout the bladder and concurrent high risk prostate cancer Gleason was 10, PSA was 10ng/ml and he had positive margin and ECE in 2022. Rad Onc? Burnt Out?
Here’s some advice from a senior medical director from a leading prior auth company. You know, those guys who make you wait one week to do a P2P because you wanted to do SRS on a solitary brain met and the nurse reviewer said she can authorize it right away if you do it “by 3D.” Then the P2P guy says, sorry prostate cancer brain mets are excluded from our SRS policy. Unusual Location PSMA Node
53 yo very healthy man with unfav int risk prostate cancer Gleason 4+3 in 12 of 16 targeted biopsy cores PSA 16 ng/ml (was 14 back in November) had a PSMA for staging that shows Suvmax uptake of 4.1 in a small perirectal node (see picture). Bone met scenario
Here's a scenario I need help with: 70ish yo M with castrate-resistant stage IV prostate cancer. PSMA PET showed a mid femur met along with other small bone lesions, the patient was asymptomatic. I radiated the mid femur met 30 Gy in 10 fractions hoping to prevent a fracture (based on the data presented at ASTRO). PSA goes down (patient is on xtandi btw). Insurance Denial of IMRT for Prostate
Anyone here run into this before? I've got a P2P tomorrow with Priority Health. They're denying IMRT for 28-fraction prostate treatment for an intemediate risk prostate cancer. They're telling me they'll only approve brachy or 3D. This is the first time I've ever had IMRT denied for prostate. It's standard of care and am frankly shocked by the denial. Person I'm talking to tomorrow is not a rad onc. They don't have a rad onc to talk to. Are other folks seeing this kind of shenanigans? (Source: Student Doctor Network)

Lymphadenectomy in high-risk prostate cancer
Hey! I am asking for a friend here. He is planning a trial and would like to have some feedback on worldwide practice. In your experience, do urologists perform lymphadenectomy in high-risk cN0 prostate cancer (provided the high-risk cancer was known before surgery - based on GS, PSA, cT3) when performing radical prostatectomy as primary treatment.
