Eyebrow Lift: Open-Book ROOF Transposition and Orbicularis –Frontalis Sliding Maneuver
This article delves into innovative surgical techniques, the open-book ROOF (retro-orbicularis oculi fat) transposition technique (OBRoof) and the orbicularis–frontalis sliding maneuver (OFS), designed for eyebrow and upper eyelid rejuvenation in the aging process. Emphasis is placed on understanding the dynamic changes occurring within the forehead and eyebrow envelope's five layers and their intrinsic relationships as well as the impact of the ROOF ptosis causing upper eyelid hooding. The OBRoof technique is highlighted for its effectiveness in addressing ptotic volume, offering a rejuvenated appearance by relocating v...

Restoring facial superfacial muscular aponeurotic system defects after permanent fillers removal using fascial and SMAS grafts: a preliminary study
ConclusionsThis study shows promising aesthetic outcomes and high patient satisfaction at 6 months after simultaneous facelift and permanent filler removal with volume defect correction using various local tissue grafts. Larger studies with longer follow-ups are warranted to further assess these reconstructive techniques for significant SMAS defects resulting from improper permanent filler placement.Level of Evidence: Level IV, therapeutic study (Source: European Journal of Plastic Surgery)

Male Facial Rejuvenation
The rise in popularity of facial cosmetic enhancement for men has been spurred on in part by increasing social media awareness. Traditionally, men have been fearful not only of looking unnatural but also for undertaking procedures that they may deem to be uncommon for their gender. However, today with online media celebrating many men revealing their natural transformations, those barriers are quickly falling away. This past year I have performed almost as many facelifts in men as in women. Conversely, I also perform almost as many hair transplants in women as in men, perhaps for similar reasons of stigmas or unawareness f...

The Nuances and Techniques of Deep Plane Face and Neck Lifting to Maximize the Defined Neck and Jawline and Obtain a Sharp and Beautiful Neck Contour
This article seeks to expand on our understanding of lifting by utilizing the benefits of deep plane release and repositioning. The deep plane is a more logical and natural approach to lifting of the face and neck, lifting along the natural anatomic glide planes. Deep plane face and neck lifting have demonstrated superior results in the authors' experience with less dependence on ancillary measures such as fat grafting or implantation for midface volumization. The deep plane technique allows for the face and neck to be treated and lifted as a single composite unit, providing a more natural and long-lasting result and less ...

Medial Ligament Release: Is It a Proportional Standard?
Facial Plast Surg DOI: 10.1055/a-2290-3904Assessing the proportionality of extended facelift surgery across surgeons is not an easy task. Risks and assumed benefits need to be weighted, yet, especially the latter is difficult to objectify. Reverse engineering the pathway of two leading facelift surgeons suggest that excellent training and focused high-volume facelift surgery preceded their leadership. Yet these conditions are not available to all surgeons. Hence defining extended facelift techniques as the professional standard, could promote novice surgeons into a path that may not correspond to safe practice. [...] Thiem...

Direct Neck Lift for Men
Direct neck lift offers an excellent surgical technique for men seeking to rejuvenate the neck and avoid a full rhytidectomy. In this chapter, we provide an overview of direct submentoplasty techniques, as well as clinical pearls to consider in the preoperative, intraoperative, and postoperative periods. Different surgical incisions and resultant scars in the anterior neck are discussed and illustrated with figures. Given the degree of variation of submental fullness with which patients present, it is beneficial to be familiar with several different techniques to address the submental and submandibular areas. Advanced Deep-Plane Rhytidectomy: Advantages for Neck Treatment and Volumization of the Midface
Facial Plast Surg DOI: 10.1055/s-0044-1785452The aged neck is the most common motivator for patients seeking facial rejuvenation. Unlike the deflated midface, surgical treatment is still the gold standard for optimal outcomes. It is our view that the majority etiology of both the aged neck and deflated midface is gravity's effects on the superficial soft tissue envelope, leading to soft tissue redundancy. It is also our view that the deep-plane rhytidectomy is supported by anatomy, embryology, and clinical evidence as the best methodology to treat soft tissue redundancy without producing the aesthetic and longevity issues ...

Male Deep-Plane Face and Neck Lifting
In this article, the authors describe their preferred advanced deep-plane techniques and modifications that have universally improved outcomes and durability in both men and women. Performing a proper extended deep-plane facelift and neck lift avoids the need to camouflage scars and stigmata of lifts seen in superficial musculoaponeurotic system plication and other techniques. In the author ’s experience, vertical vector deep-plane surgery is more durable, natural, and less reliant on lipofilling and volume addition. The subtleties of examination and analysis, surgical technique, clinical outcomes, and gender-specific co...

Advanced Facial Rejuvenation: Synergistic Effects of Lower Blepharoplasty and Ultrasound Guided Mid-Face Lift Using Polydioxanone (PDO) Threads
ConclusionThe combination of lower blepharoplasty and mid-face lift using PDO threads is a comprehensive and effective approach for facial rejuvenation. It significantly enhances wrinkle reduction, mid-face lifting, and patient satisfaction. Ultrasound-guided thread lifting, a method of assessing and performing mid-face lifting, proves to be safe and efficient. This approach holds promise as a future option in cosmetic anti-aging surgery, presenting a minimally invasive alternative with natural-looking results and reduced downtime.Level of Evidence IIThis journal requires that authors assign a level of evidence to each art...

The Detailed Anatomy of the Deep Plane of the Face and Neck with Current Nomenclature
Facial Plast Surg DOI: 10.1055/a-2268-6735The fundamental goal of modern face and neck lifting is to enhance and restore a more youthful facial appearance by addressing soft tissue descent. There are variations in described facelift technique including superficial musculoaponeurotic system (SMAS) flaps, composite flaps, deep plane, skin flaps, and subperiosteal facelifts, among others. The term “deep plane rhytidectomy” was originally described by Hamra. This term has since been linked with facelift surgery with recent advances including incision placement, treatments of SMAS–platysma complex, and mastoid crevasse am...

Unilateral Platysma Muscle Rupture as an Effect of Using a Hard Wooden Block for Facial Massage
Arch Plast Surg. 2024 Feb 7;51(1):27-29. doi: 10.1055/a-2175-8052. eCollection 2024 Jan.ABSTRACTFacial massages are frequently performed to achieve a feeling of freshness, rejuvenation, skin tightening, and delayed onset of wrinkles. However, vigorous massages can induce unexpected symptoms. Here, we present a case of a woman who complained of an asymmetric facial appearance and a mass-like lesion following a long-term facial massage intervention. A facelift incision was performed. Platysma muscle rupture was observed intraoperatively, which was then repaired. To our knowledge, this is the first report of a vigorous facial...
