Why do people with pain report differently on questionnaires than they do in physical performance testing?
One of the topics thrown around by people who don’t have an up-to-date understanding of pain is why people say one thing on a questionnaire, for example, what they can and can’t do, and perform quite differently when asked to do the same task in a clinic. It’s a good question, on the face of it: after all, people should know what they can and can’t do, and be consistent. If there is a difference, well obviously the physical performance test is far more objective than self-report – the therapist is right there watching, so there’s no room for doubt about which measure is The Most Accurate...

Jeffrey Miron and Pedro Braga SoaresAmazon recentlyannounced that it will no longer include marijuana in the company ’s drug screening program for new hires (other than for positionsregulated by the Department of Transportation). Amazon says marijuana will instead be treated like alcohol, which means “impairment checks” on the job site and testing for all drugs and alcohol after job incidents.Drug screening and testing arewidespread in the US, covering nearlyhalf of all workplaces. But Amazon ’s announcement, and the recent spate ofmarijuana medicalizations and legalizations, have prompted discussion of who should ...

In search of an evidence-based approach to occupational therapy practice education that would include simulation experiences
Simulation is a methodology used to replicate real-world tasks.  Simulation provides experiences that lead to direct learning or that can be used as an assessment of competence.  Such tasks have been used as a primary tactic in occupational therapy and other rehabilitation services since their inception - although they were generally applied in a therapeutic context.  For example, an individual would learn how to dress themselves out of context to develop skill, and then that skill would be transferred to contextual learning.  Sometimes, more esoterically, a simulated activity broken down into com...

Interrupting the Medical Ableism Feedback Loop
by Joseph Stramondo, PhD The COVID-19 pandemic reveals systemic health inequities in ways that are hard to ignore.  These inequities tracked race, class, and several other overlapping and interlocking oppressions, including ableism. Ableism roughly refers to an ideology that disrespects and unjustly disadvantages disabled people on account of their disability.  Ableism can appear in the attitudes and beliefs of individual people or it can be “baked in” to a system of institutions and practices.   Systemic ableism was most apparent early on in the pandemic, in the way cris...

The New Deal and Recovery, Part 13: Fear Itself (Continued)
George Selgin(This post continuesmy discussion of the " regime uncertainty " hypothesis, according to which theNew Deal hampered recovery by causing businessmen to fear policy changes that might render their investments unprofitable.)Insull ' s MonstrosityThe 1935 Revenue Act wasn ' t the only measure that had businessmen and investors shuddering that August. Less than a week after it became law, FDR signed the still-more controversialPublic Utility Holding Company Act, granting the SEC the power to break up the nation ' s utility holding companies. On the eve of the Depression,Paul Mahoney explains, most U.S. electric and ...

First Brain-Computer Robotic Stroke Rehab System Authorized by FDA
Neurolutions, a company with offices in Santa Cruz, CA and St. Louis, MO, won FDA de novo authorization to introduce its IpsiHand stroke rehab system in the United States. This is the first approval of a brain-computer interface technology for the rehabilitation of stroke patients, and it promises a faster and more complete recovery of hand function in many patients. The IpsiHand system takes advantage of the uninjured side of the brain to trigger the opening and closing of a robotic exoskeleton placed over the affected arm. The patient wears a brain-wave detecting EEG headset that interprets the relevant signals a...

Sympathy for the Devil
As the aptly named Mr. Chauvin awaits sentencing, I get the impression that many people think he will get a much longer sentence than is likely to be the case. I ' ve read up on this a bit and while the statute in Minnesota allows up to 40 years for second degree murder, in fact the guidance for a first time offender is only 12 1/2 years.  Lest you be too outraged, the definition of the crime for which he was convicted does not include that he intended to cause death. (About that question we cannot know.) Since there are hints the judge is sympathetic to him, it ' s doubtful he ' ll impose a longer sentence than ...

What Would The Ideal Hospital Look Like?
If your imagination could run wild, how would you envision the future of hospitals? Hi-tech big machines, physicians analysing data obtained from patients’ devices, LED screens greeting visitors by their names using facial recognition, virtual patient visits, and robots leading operations… Well, experts have pretty much the same idea. Recently we wrote a lot about the future of hospitals. We emphasised the importance of good design both inside and outside the point of care. We imagined that these institutions will become “health centres for patients for preventing diseases, for acute care patients and for patient...
