Fetal aortic valvuloplasty
is considered for fetuses with severe valvar aortic stenosis and echocardiographic features suggesting a risk of progression to hypoplastic left heart syndrome. Though surgical options are available for infants with hypoplastic left heart syndrome, morbidity and mortality are high. So if progression to hypoplastic left heart syndrome can be prevented by fetal aortic valvuloplasty, that would be theoretically a great boon. But centers which offer fetal aortic valvuloplasty are very few and cases suitable for fetal aortic valvuloplasty are quite rare. Hence there is difficulty in attaining good case volumes for optimal proc...

Role of ultrahigh-spatial-resolution photon-counting CT in assessment of coronary artery disease
I had posted about photon counting CT which  converts X-rays directly to electronic signals unlike conventional CT which converts X-rays first to light and then to electronic signals. Direct  conversion allows low energy photons to be differentiated from electronic noise. When data is acquired directly from photons of different energy levels, better contrast-to-noise ratio can be realized. Photon-counting-detector CT also improves spatial resolution. A study published in Radiology, the flagship journal of the  Radiological Society of North America evaluated both in vivo and in vitro coronary stenosis using ultrahigh-sp...

Will Patients Have To Pay For Using AI In Their Healthcare?
This article discussed how the author was asked if she wanted to pay $40 extra for additional AI analysis in mammography. In her case a Manhattan radiology clinic offered an AI analysis of their mammogram for an additional $40, not covered by insurance. This scenario was echoed at a clinic in suburban Baltimore, where patients were similarly offered AI-assisted mammography for a $40 fee. These instances mark the initial real-world applications of AI in patient care but also introduce new factors to the healthcare equation.  To make things more complicated, we can’t look for a single, universal solution here. Heal...

A teenager involved in a motor vehicle collision with abnormal ECG
Written by Pendell MeyersA teenager was involved in a motor vehicle collision and presented to the Emergency Department via EMS altered and potentially critically ill. He was intubated for altered mental status. Chest trauma was suspected on initial exam. Here is his initial ECG around 1330:What do you think? The ECG shows sinus tachycardia with RBBB and LAFB, without clear additional superimposed signs of ischemia. It is very unlikely that a previously healthy teenager would have such disease of the conduction system, bringing up the possibility of blunt cardiac injury in this clinical setting.Trauma CTs showed a " mi...

Digital Health at CES 2024
CES is always a fun show for a tech guy like myself.  With 130,000 people, there’s a lot of noise that doesn’t relate to healthcare IT, but over the years the digital health portion of CES has really grown.  This year that growth was particularly true thanks to the great efforts of Catherine Pugh who put together an entire Digital Health track, a long list of digital health vendors, and a packed house digital health networking event. While I’m sure I missed plenty of the digital health happenings at CES, here’s a quick rundown of some of the things I found interesting. Digital Health Media Panel a...

Orthostatic hypotension onset after invasive procedure?
Written by Willy FrickA man in his 70s with a history of HFrEF and sick sinus syndrome s/p dual chamber pacemaker placement was admitted for overnight observation following outpatient placement of a mitral valve clip. The procedure note indicates uncomplicated clip placement. The next morning, the following ECG was obtained. What do you think? The tracing shows sinus rhythm with PVCs and non-specific ST&T wave abnormality. But there is something more important to notice, which is the pacer spikes. They do not make sense. Some of them are in the middle of or after P waves, and there ' s even one that falls at the end...
