Safety and adverse events during primary care paramedic interfacility transfer of stable STEMI patients
CONCLUSIONS: We found PCP-interfacility transport of initially stable STEMI patients was safe and associated with a moderate proportion of adverse events, the majority of which did not require an advanced care intervention. These findings may help decision-making to avoid delays transferring stable patients to PCI-capable centers.PMID:38619868 | DOI:10.1080/10903127.2024.2342569 (Source: Prehospital Emergency Care)

Group A Streptococcus Carriage in Adults in Southern Israel: A Prospective Study
CONCLUSIONS: Being an ultra-Orthodox Jew was the single variable associated with a GAS carriage, which may be related to having many children at home and living in overcrowded areas. Primary care physicians in Israel should recognize this situation when examining patients with sore throats, mainly ultra-Orthodox Jews.PMID:38616668 (Source: The Israel Medical Association Journal)

Diarrhea and HIV infection
Med Mal Infect. 2003 Feb 1;33:105-110. doi: 10.1016/S0399-077X(02)00479-1. Epub 2003 Feb 19.ABSTRACTThe development of Highly Active Antiretroviral Therapies and of protease inhibitors in particular has permitted in the last ten years to dramatically improve the viral replication in HIV infected patients but the incidence of side effects and of diarrhoea in particular remains high. In general practice, the occurrence of diarrhoea should systematically lead to eliminate an infectious ætiology of diarrhoea. Many studies have demonstrated that diarrhoea under protease inhibitors and nelfinavir in particular are of a low grad...

Respiratory physiotherapy in post-COVID-19: a decision-making algorithm for clinical practice
Open Respir Arch. 2021 Oct 12;4(1):100139. doi: 10.1016/j.opresp.2021.100139. eCollection 2022 Jan-Mar.ABSTRACTThe outbreak of COVID-19 has posed a great challenge for the healthcare system which has been later aggravated by the need of managing clinical manifestations and potential sequelae in COVID-19 survivors. In this context, respiratory Physiotherapy emerges as a cornerstone in the interdisciplinary management warranted in this population. Given that the implementation and resources available for the interdisciplinary therapeutic interventions in Spain is scarce, it is essential to perform a comprehensive, exhaustive...

Gastroenterology/Hepatology: What You May Have Missed in 2023
This article summarizes clinically important gastroenterology developments from 2023 for internal medicine specialists. In colorectal cancer screening, a new RNA fecal screening test is on the horizon, as well as a new analysis on the benefits of using artificial intelligence in screening colonoscopy to detect more polyps. There is new evidence for management of gastrointestinal bleeding, a new drug for treatment of recurrent small-intestinal angiodysplasia, and a new endoscopic treatment method in patients with gastrointestinal tumor bleeding. The authors feature a randomized trial about amitriptyline as treatment for pat...

Reducing CVD health disparities in Black women through addressing social determinants of health (SDOH): a scoping review
Women Health. 2024 Apr 15:1-15. doi: 10.1080/03630242.2024.2337719. Online ahead of print.ABSTRACTCardiovascular disease (CVD) disparities among Black American women can be linked directly to social determinants of health (SDOH). This scoping review examines the breadth and depth of existing literature on CVD risk reduction interventions in young-to-middle-aged women that address SDOH. We searched PubMed, CINAHL, Scopus and Google Scholar for relevant peer-reviewed articles published in English. We included studies if they reported on the feasibility, acceptability, or findings of a CVD risk reduction intervention, address...

Impact of Ramadan fasting on lipid profile, uric acid, and HbA1c in CKD: A systematic review and meta-analysis
This study aims to conduct a comprehensive evaluation of studies related to the effect of fasting in the holy month of Ramadan on lipid profile, uric acid, and HbA1c in CKD patients. Studies were systematically searched and collected from three databases (PubMed, Scopus, and Web of Science). After screening, the quality and risk of bias assessment of the selected articles were evaluated. Study heterogeneity was assessed using the Cochrane test and I² statistic. In case of any heterogeneity random effects model with the inverse-variance method was applied. All analyses were performed using STATA software version 16. Four o...

A multimodal approach using fundus images and text meta-data in a machine learning classifier with embeddings to predict years with self-reported diabetes - An exploratory analysis
CONCLUSIONS: The machine learning model had acceptable accuracy and F1 score, and correctly classified more than half of the patients according to diabetes duration. Using large foundational models to extract image and text embeddings seems a feasible and efficient approach to predict years living with self-reported diabetes.PMID:38616442 | DOI:10.1016/j.pcd.2024.04.002 (Source: Primary Care)

Effectiveness of tailored talks between a cancer screening specialist and general practitioners to improve the uptake of colorectal cancer screening in Ancona (Italy) during the pandemic period
CONCLUSION: Persons cared for by GPs who received a tailored talk with a cancer screening specialist avoided a drop in CRC screening adherence, which characterised all other Italian screening programmes during the COVID-19 emergency. If future randomised trials confirm the impact of tailored talks, they may be incorporated into existing strategies to improve population CRC screening uptake.PMID:38618885 | DOI:10.1080/13814788.2024.2340672 (Source: European Journal of General Practice)
