Mitigating risk of pregnancy and fluoro
Curious what others are doing. While back did a hip injection under fluoro, patient denied any chance of being pregnant and we routinely record date of last menstrual period. Two days after the injection she has a positive pregnancy test. Her mgy was very low and the risk is minimal, but curious what others do to prevent this from happening. Day of procedure pregnancy test in women of child-bearing age? Avoid fluoro during certain times of cycle? Depend on patient history? (Source: Student Doctor Network)

Family-friendly Family Medicine Residency Programs
I hope to have a newborn and/or will be pregnant during my intern year. I am in my mid/late-30s so waiting is not much of an option, unfortunately. What are some things that I can ask about or look out for while I am interviewing for Family Medicine residency programs? Are there some things that make some programs more family-friendly or better for new moms/parents during internship/residency? How do you go about asking these questions during interview season? Or how do you go about... Read more (Source: Student Doctor Network)

Endocrine fellowship chances... new mom
Hi.. I am currently a PGY 2 Medical Resident who is pregnant and due to deliver in June 2021. I would be likely taking a 4 week maternity leave, and during the pregnancy had a 4 week break due to medical issues. What would be the best advice on applying for the 2022- endocrinology fellowship intake considering that I would have an extension of residency by at least 8 weeks ? Is it even realistically possible for programs to offer and make changes in schedule even if there is a delay in... Read more (Source: Student Doctor Network)

OB case
Saw this last week, curious how y'all would have approached. 26F h/o meth and etoh abuse showed up to maternity clinic at 36w pregnant with dyspnea no prenatal care prior. Had TTE done in clinic showing EF 30% and was instructed to admit for further workup but declined. A week later showed up by EMS with hemoptysis, given therapeutic lovenox in ER (no idea why), CTA showed pulmonary edema, admitted to ICU for SROM and active labor 4cm dilated station 0, contractions every 3-4 minutes... Writing about / oversharing struggles in PS?
Hey guys, One of the first things that made me want to pursue medicine are some experiences I had growing up. However, some experiences were shared to me by my mom as they happened when I was a baby. I'll quickly summarize: Mom came from Central America, met my "dad" and my sister was born. I was born 2 years later but my "dad" abused my mom physically and injected heroin into her while she was pregnant with me. She went to the doctor to discuss this and they let her know that I was... Would being pregnant going into intern year have changed your ranking of pediatrics programs?
Hey SDN, So, hypothetically, say one were to have a positive pregnancy one week before their rank list for pediatrics was due, would this information have changed the way in which you ranked programs? All the programs where you applied were medium to large residencies (>20 residents) and many of them are at academic centers and known to be intense places that work their residents hard. On the interview trail every place said they were family friendly, and then in the next breath recommended... Read more (Source: Student Doctor Network)

Rhesus Disease
Good morning / day / evening, I realise this is a very specific question, and thus I'm not surprised to have not read about it in my Ob & Gyn book. If a woman were to receive a blood transfusion which contained Rh +ve blood, but she is Rh -ve, would she require earlier shots of Anti-D if she becomes pregnant in the future? Thank you! What are my chances / next step
Hello. I need advice on my next steps to matriculation into medical school. I am a first generation URM (black male). Father finished high school only, mother did not finish high school due to teenage pregnancy. Grew up in poverty. I don’t need anyone to sugarcoat anything, just give me the hard facts. I will be a non traditional applicant in the sense that it took me 6 years to graduate with a biology degree and a double minor in psychology and sociology. My first 3 and a half years in... Broken epidural catheter
Hello. This question was asked by someone before. 36 weeks pregnant, expecting a labor epidural, saying had an epidural catheter broken in her back many years ago and was not retrieved. no other history available and presumably asymptomatic. What would be a reasonable approach? my colleagues say "Just do it and if something happens it's your fault". Thanks. (Source: Student Doctor Network)

Pregnancy planning in fellowship
Hi all! I’m a third year FM resident doing a one year surgical OB fellowship next year. I am getting older and my husband and I want multiple kids (probably 3+). I’ll be 30-31 during fellowship year. The fellowship schedule is strenuous (q2 home call for c-sections, one 30 hour call monthly, one period of working two weeks on per month). It’ll be stressful—new city, new skills, new responsibilities. But, I need to start trying for a baby soon before it’s too late. My top priority is having a...
