High copy number polymorphisms of alpha defensin 1-3 are associated with therapeutic responsiveness in inflammatory bowel disease
Prednisolone therapy is the first choice for the treatment of acute severe ulcerative colitis (ASUC), however, the refractory case is sometimes observed. Alpha defensin 1-3 (DEFA1A3) stored in neutrophils acts as an effector factor of innate immunity. It has been reported that the copy number variation of the DEFA1A3 gene is associated with the severity of ulcerative colitis (UC). (Source: Gastroenterology)

A case of attempted transbronchial spigot insertion for fistulous pyothorax in the residual pleural airspace after pleurectomy/decortication for malignant pleural mesothelioma
Herein, we report a case of attempted transbronchial spigot insertion for fistulous pyothorax in residual thoracic airspace after pleurectomy/decortication for malignant pleural mesothelioma. During attempted transbronchial occlusion of the responsible bronchus, some spigots penetrated the empyema cavity, and open window thoracotomy was performed on the following day. AbstractPleurectomy/decortication for malignant pleural mesothelioma is a relatively recent surgical approach for which there is a dearth of information on complications, especially in the late postoperative period. A 70-year-old man was diagnosed with right ...

Azathioprine-induced Veno-occlusive Hepatotoxicity in a Patient with Myasthenia Gravis
CONCLUSION: A possibility of AZA veno-occlusive hepatoxicity should be considered in an MG patient if presented with abdominal pain, elevated bilirubin and transaminases and ultrasound showing dilatation of hepatic veins. Physicians should be aware of this complication because this toxicity is reversible following dose reduction or withdrawal of AZA.PMID:38265400 | DOI:10.2174/0115748863272041231116104839 (Source: Current Drug Safety)

Drug ‐induced lung injury in a patient treated with prior atezolizumab and subsequent sotorasib: A case report
We report a case of drug-induced lung injury treated with prior atezolizumab and subsequent sotorasib. Definitive identification of the responsible drug for the drug-induced lung injury proved challenging in the setting of exposure to multiple potential inciting agents. There is a need for high levels of clinical suspicion for timely evaluation and management. AbstractWe report a case of drug-induced lung injury treated with prior atezolizumab and subsequent sotorasib. The patient was a 62-year-old woman with lung adenocarcinoma harbouring a KRAS G12C mutation that was resistant to chemotherapy, including immune checkpoint...

Cancers, Vol. 16, Pages 508: Enzalutamide versus Abiraterone Plus Prednisolone for Nonmetastatic Castration-Resistant Prostate Cancer: A Sub-Analysis from the ENABLE Study for PCa
hei Hashimoto Takashi Fukagai Kazuyoshi Shigehara Shizuko Takahara Yoshifumi Kadono Atsushi Mizokami Enzalutamide (ENZ) and abiraterone plus prednisolone (ABI) can improve the survival of patients with castration-resistant prostate cancer (CRPC). However, the agent that is more effective against nonmetastatic CRPC remains unclear. To evaluate the agent that can be used as the first-line treatment for CRPC, an investigator-initiated, multicenter, randomized controlled trial (ENABLE Study for PCa) including both metastatic and nonmetastatic CRPC was conducted in Japan. The prostate-specific antigen (PSA) respon...

Average daily glucocorticoid dose, number of prescription days, and cumulative dose in the initial 90  days of glucocorticoid therapy are associated with subsequent hip and clinical vertebral fracture risk: a retrospective cohort study using a nationwide health insurance claims database in Japan
ConclusionsGC exposure in the initial 90  days of GC therapy was dose-dependently associated with hip and clinical vertebral fracture risk. GC may increase fracture risk with lower doses for shorter durations than previously reported.SummaryFracture risk assessment three months after glucocorticoid (GC) therapy initiation is recommended. We found that GC exposure in the initial 90  days of GC therapy at lower daily doses for shorter durations than previously reported were significantly and dose-dependently associated with fracture risk using a nationwide health insurance claims database. Obesity and its Relationship with Covid-19: A Review of the Main Pharmaceutical Aspects
Curr Pharm Biotechnol. 2024 Jan 22. doi: 10.2174/0113892010264503231108070917. Online ahead of print.ABSTRACTImportant physiological changes are observed in patients with obesity, such as intestinal permeability, gastric emptying, cardiac output, and hepatic and renal function. These differences can determine variations in the pharmacokinetics of different drugs and can generate different concentrations at the site of action, which can lead to sub therapeutic or toxic concentrations. Understanding the physiological and immunological processes that lead to the clinical manifestations of COVID-19 is essential to correlate ob...

New ‐onset ANCA‐associated vasculitis presenting with neuropathy after COVID‐19 infection: A case report and literature review
This study discusses a case with neuropathy and positive ANCA after COVID-19 infection and reviews the literature on AAV following COVID-19 infection. A 59-year-old man is presented that was referred to Shariati Hospital for evaluation of neurologic problems after a COVID-19 infection. Initially, he had flu-like symptoms. A few days later, he developed right distal upper and lower limb paresthesia. His electromyography (EMG) and nerve conduction velocity (NCV) results were consistent with polyneuropathy. Lumbar puncture (LP) was normal except for positive COVID-19 polymerase chain reaction (PCR). The patient's paresthesia ...

In sarcoidosis trials, time also matters
Extract Sarcoidosis is a complex and heterogeneous disease for which there are very few prospective and randomised trials available. We read the article by Dhooria et al. [1] with great interest, as it addresses significant important questions not (or rarely) reached until now. First, it compares two treatment regimens with different glucocorticoid dosages. Second, it focuses on a significant number of patients with an intermediate risk but impaired quality of life. Third, it includes various quality of life scores as key secondary outcomes. Consequently, this article fills some gaps identified in the recent European Respi...

Follow the stars: blood eosinophil-directed treatment of COPD exacerbations Oral corticosteroids (OCS) have been the acute treatment for COPD exacerbations for decades based on small trials primarily conducted in inpatients. But OCS have devastating long-term risks that increase cumulatively with dose. Ramakrishnan and colleagues (Lancet Respiratory Medicine 2023, DOI: 10.1016 /S2213-2600(23)00298-9) tested whether blood eosinophil-directed prednisolone treatment (BET) was non-inferior to standard treatment with prednisolone (ST) in this multicentre, double-blind, randomised, placebo-controlled trial (STARR2). In 14 UK-bas...
