What was Mr. Gower doing with a big jar of poison?
I watched It's a Wonderful Life last night. I hadn't seen it in several years and was amazed at how good a film it was. But something is bothering me: What was Mr. Gower doing with a big jar of poison in the pharmacy? I can understand keeping poisonous drugs on hand (strychnine had all kinds of medicinal uses), but why not have that on the label? (Source: Student Doctor Network)

Would this count towards BPCM?
Does Toxicology count towards BCPM? This is the course description, "serves as an introduction to the fascinating world of chemical poisons and covers their numerous and varied effects on human health and the environment. We will learn how and why poisons have played an important history, how to critically evaluate the chemical risk information reported in the media, in addition to the underlying principles of the basic science of poisons." Interesting case
Thought this was an interesting case, and figured I'd post it. Zero pain complaints, just weakness. Early 60s F professor, husband is a surgeon. I got an email from him a few weeks ago asking if I'd see his wife who has lower extremity weakness. Severe food poisoning (2009) with weakness, myalgias, rhabdo, progressive LE wasting and lower extremity weakness. Saw a local PMR guy who has a great reputation maybe months after this incident or possibly a year later or so, and diagnosed with a... Read more (Source: Student Doctor Network)

Maintaining knowldge
Hey all, Ive been out of residency for a year, just working in the community, 15-20shifts a month- never had any issues. I started studying for oral boards and I realized I forgot medicine. I did not remember iron overdose is treated by deferoxamine or indications. Did not remember all the meds needed to be given in acute angle glaucoma...both of those easily looked up and helped with Poison Control/Optho. Am I a bad doctor? I went to a good residency, I feel more than prepared, this is... Read more (Source: Student Doctor Network)

Is tox fellowship worth it ?
No tox specific job ...unless you are lucky to be a medical director of a poison center . Couple of my freinds regretting doing toxicology fellowship . they are thinking if they would have done critical care or EMS or sports medicine, they could have been in much better position (Source: Student Doctor Network)

Dying doesn ’t need physician assistance but a legal process to dispense lethal poison in circumscribed settings which would protect patients and doctors whilst respecting autonomy and the public’s wish

Can I prescribe my girlfriend zofran?
I am a PGY3 internal medicine resident who's about to graduate in 2 months. My girlfriend and I are planning a vacation to Tulum, Mexico, where tap water is not drinkable. I want to prescribe zofran to my girlfriend so that in case we run into food poisoning/etc, we have zofran to take at the very least. Is there any way I can get in trouble for this? I am still under a hospital issued NPI number. (Source: Student Doctor Network)

Oregon Health and Science University (OHSU) Residency Reviews
After visiting the program, I really was convinced that the rumors I had heard about low volume and lack of procedures at OHSU were false. I went and read the reviews at scutwork and found this one posted in 3/2006 that resurrected those fears. Anyone from the program care to respond. Here is the review posted below. Schedule This program had a very easy workload. 12-13 eight hour shifts over the month which includes a ride-along with paramedics and a couple hours at the poison... Poison Control required consult in your ED?
One of my pet peeves is the statement "oh you are required to do this or that" due to ED or hospital protocol, etc. But then you ask to see the policy and it ends up being bull**** that has just been passed down from generation to generation despite it not making sense. The ED Nurse Manager just told me that when things like mushroom ingestion, envenomation from snakes or other animals, etc comes in that we are required to call poison control which I have always known not to be true and... Iron studies in lead poisoning?
There's a question in UWorld where a lead poisoning patient has normal serum iron studies. In the explanation it says "(In lead poisoning) Iron studies are normal unless co-existing iron deficiency is present as well". Does anyone have any idea why that is? I was under the impression that - like all sideroblastic anemias - a lead poisoning patient would have increased serum iron, increased ferritin and normal to low TIBC. Goljan also lists iron studies in lead poisoning as having a... Iron studies in lead poisoning? (Source: Student Doctor Network)

Diamox overdose
Glaucoma patient for trabeculectomy. Actually only found out postop when nurse noticed pt was odd. Altered cognition, imbalance, other non-focal neuro deficits Had a mild AKI from diuresis, bicarb low, LFTs ok Notably related onset of symptoms to within a day of starting diamox Also determined to be taking at least 2x (perhaps 4x) the prescribed dose -- immediately suspected OD, with much lower suspicion for ddx including cerebellar stroke Poison control center confirmed ssx c/w diamox...
