A Meta-Analysis on the Global Prevalence, Risk factors and Screening of Coronary Heart Disease in Non-alcoholic Fatty Liver Disease
Cardiovascular disease remains the leading cause of death in patients with non-alcoholic fatty liver disease (NAFLD). Studies examining the association of coronary heart disease (CHD) and NAFLD are cofounded by various cardiometabolic factors particularly diabetes and body mass index. Hence, we seek to explore such association by investigating the global prevalence, independent risk factors and influence of steatosis grade on manifestation of CHD among patients with NAFLD. We appreciate the comments by Akiyama et  al1 regarding our recent publication evaluating the “Incidence, Risk Factors, and Outcomes of Pouchitis and Pouch-Related Complications in Patients with Ulcerative Colitis.”2 Given the authors’ recent findings of a high risk of developing pouch inflammation over long-term follow-up,3 and pri or long-term studies indicating the risk of pouchitis may be as high as 80%,4 we believe that these studies should continue to prompt future investigations into the underlying drivers of pouchitis and other inflammatory conditions of the pouch. We thank Dr. Jena and colleagues for their interest in our study1 and for their insightful comments. As they have indicated, we know from considerable data2 that difficult cannulation is associated with postendoscopic retrograde cholangiopancreatography pancreatitis (PEP), and that inadvertent pancreatic duct cannulation/guidewire insertion and long cannulation time (ie, difficult cannulation) are key risk factors for PEP.3 Although we did not acquire detailed information regarding how many times and for how long the fellow/trainee attempted cannulation, data for the overall cannulation times and unintended pancreatic duct...
