Duodenal Mucosal Barrier in Functional Dyspepsia
In addition to gastric sensorimotor dysfunctions, functional dyspepsia (FD) is also variably associated with duodenal micro-inflammation and epithelial barrier dysfunction, the pathogenesis and clinical significance of which are unknown. Distinctive Clinical Correlates of Small Intestinal Bacterial Overgrowth with Methanogens
Most patients with small intestinal bacterial overgrowth (SIBO) produce hydrogen by fermentation of dietary carbohydrates; however, ∼30% of patients with SIBO are colonized with Archaea, anaerobic organisms that produce methane. SIBO is associated with a plethora of symptoms and conditions, but their diagnostic significance is unclear. We thank Lv et  al1 for their interest and comments in our study. Underwater endoscopic mucosal resection (UEMR) is recently getting attention for its efficacy and safety for treatment of superficial nonampullary duodenal epithelial tumor (SNADET). Usefulness of UEMR for SNADET had been reported in several report s; however, previous studies had small sample size without evaluation of clinically relevant end points.2,3 Thus, we conducted a multicenter prospective study to evaluate the nonrecurrence rate of UEMR for SNADET using careful follow-up biopsy. We thank Dr Jindal for his interests in and comments on our study, which we address herein. First, not all chronic hepatitis B (CHB) patients with cirrhosis were indefinitely reimbursed for antiviral therapy by the national health insurance in Taiwan. For patients whose viremia did not exceed 2000 IU/mL or those without overt features of portal hypertension, the reimbursement was finite (up to 3 years in general) despite a clinical diagnosis of cirrhosis throughout the study period.
