Screening for Esophageal Adenocarcinoma: Should We Use Barrett ’s Screening Protocols?
We read with great interest the recently published article by Sawas et  al,1 evaluating the limitations of heartburn and other societies’ criteria in Barrett’s esophagus (BE) screening for detecting de novo esophageal adenocarcinoma (EAC). The authors reported that 54.9% of patients in the Mayo Clinic cohort and 38.9% in the UK cohort of EAC did not fulfill the Am erican College of Gastroenterology or the British Society of Gastroenterology criteria for BE screening, respectively. Clinical Updates in Colorectal Endoscopic Submucosal Dissection
Laterally spreading tumors (LSTs) are increasingly encountered with the uptake of colorectal cancer screening programs. The approach to LSTs is determined by the risk of submucosal invasive cancer (SMIC), because this dictates the most appropriate endoscopic or surgical intervention. In the West, endoscopic mucosal resection (EMR) is the first-line treatment for LSTs without SMIC, given its cost-effectiveness, superior safety profile, and patient quality of life when compared with surgery.
