Exosomal circRNAs in the plasma serve as novel biomarkers for IPF diagnosis and progression prediction
Idiopathic Pulmonary Fibrosis (IPF) is a type of chronic interstitial pneumonia, often fatal, with elusive causes and a bleak prognosis. Its treatment options are limited and largely ineffective. Early detecti... (Source: Journal of Translational Medicine)

Management and documentation of pneumonia - a comparison of patients consulting primary care and emergency care
CONCLUSIONS: Vital signs were documented less often in primary care than in emergency care. Patients in primary care appear to have a less severe pneumonia, indicating attendance to the correct care level. The traffic light scoring model identified more patients at risk of severe infection than CRB-65, where the parameters were documented to a limited extent.PMID:38459974 | DOI:10.1080/02813432.2024.2326469 (Source: Primary Care)

Long-term hospital care needs after Bothrops atrox envenomation with hemorrhagic stroke in the Brazilian Amazon: 'From social to physical death' - A case report
We report an unusual case of a patient who evolved with an intracranial hemorrhagic stroke and was in a coma for more than five years in a tertiary hospital located in Manaus, Amazonas. 52-year-old man, carpenter, resident in the rural area of the municipality of Tabatinga, located 1106 km from Manaus, capital of Amazonas, Brazil, victim of an accident involving Bothrops atrox evolution with cardiorespiratory arrest, acute kidney injury and hemorrhagic stroke. After 43 days of hospitalization in the ICU, he was transferred to the ward, without contact with the environment and family, sent for home treatment, however, witho...

Late-Onset Diffuse Lung Disease in an 8-Year-Old Girl
Chest. 2024 Mar;165(3):e71-e74. doi: 10.1016/j.chest.2023.08.005.ABSTRACTAn 8-year-old girl presented with a 34-day history of cough, fatigue, and impaired exercise tolerance. She experienced cyanosis on exertion but denied fever, hemoptysis, hematuria, or seizures. Her perinatal and family histories were unremarkable, and she had no history of exposure to TB. A chest radiogram from a local clinic showed diffuse pulmonary lesions. Tuberculin skin test, interferon-γ release assay, and HIV antibody test results were all negative. Immunoglobulin levels and lymphocyte subsets were normal. The patient did not respond to IV azi...

Drug-Gene Risk Stratification in Patients with Suspected Drug-Induced Interstitial Lung Disease
ConclusionsIn our cohort with suspected DI-ILD, 79% carried one or more genetic variants accompanied by the use of drugs metabolized by a corresponding affected pathway. In 60%, the diagnosis of DI-ILD was likely, whereas in 37.5%, it was highly likely, based on CYP analyses. This study underlines the importance of considering both drug use and genetic make-up as a possible cause, or at least a contributing factor, in the development and/or progression of fibrotic lung diseases.Clinical Trial RegistrationClinicalTrials.gov identifier NCT00267800, registered in 2005. Clinical characteristics of patients with connective tissue disease –related interstitial lung disease: a retrospective analysis
ConclusionsAs a critical manifestation of connective tissue diseases, immunosuppressive treatment is indispensable in the management of interstitial lung diseases especially those at an increased risk for progression. The treatment approaches should be assessed in a patient-based way. The patients under immunosuppressive treatment should be cautiously followed for infections.Key Points•Interstitial lung disease is a noteworthy manifestation of connective tissue diseases.•The clinical findings, treatment requirements, and progression vary according to the severity of the disease.•Immunosuppressive treatment may be ess...

Diagnostic predictability of serum miR-4793-3p and miR-1180-3p expression in community-acquired pneumonia
Conclusion: Increased levels of miR-4793-3p and miR-1180-3p may indicate severe pneumonia development. Plasma miRNA profiling enables early prediction of severe CAP, aiding therapeutic decisions.PMID:38456294 | DOI:10.2217/bmm-2023-0463 (Source: Future Medicine: Biomarkers in Medicine)

Interferon autoantibodies as signals of a sick thymus
Front Immunol. 2024 Feb 22;15:1327784. doi: 10.3389/fimmu.2024.1327784. eCollection 2024.ABSTRACTType I interferons (IFN-I) are key immune messenger molecules that play an important role in viral defense. They act as a bridge between microbe sensing, immune function magnitude, and adaptive immunity to fight infections, and they must therefore be tightly regulated. It has become increasingly evident that thymic irregularities and mutations in immune genes affecting thymic tolerance can lead to the production of IFN-I autoantibodies (autoAbs). Whether these biomarkers affect the immune system or tissue integrity of the host ...
