Question on weight-loss drug dosage.
Gettings, We have a patient who gets Phentermine 37.5 mg BID # 60 each month. My question is, is 75 mg/ day justified? I can’t find it anywhere in package insert to go above 37.5 mg (although we have some other folks on 1.5 tab of 37.5 mg qd). Thanks (Source: Student Doctor Network)

Stimulants
Ran across this article while doing some research on phentermine written by Dr. Tennant from earlier this Summer. I have been having more requests for stimulants for fatigue, especially my central sensitization crowd and post-covid crowd. I haven't had requests for them for pain. Anyone else seeing an uptick? Phenteramine ..again
Do you guys postpone or cancel cases if patient is on phentermine?? I recently had a total knee replacement tell me she started taking it 2 weeks prior to surgery, I have done short case where young people have been on phentermine and had hypotension that was prolonged so i have been hesistant to do a case where phentermine has been on board. For the state of NJ, is it possible to ask the board of pharmacy legal questions and get an answer? I feel like the contact us section is more for application based questions. Can you fill a control medication for a greater day supply than 30 or 90? I had a phentermine RX recently that called for 1/2 tablet three times a week, quantity 30. Phentermine Dosing of 37.5mg BID
Had an interesting situation pop up today when it came to an Adipex prescription where I got a script for 37.5mg BID. Got into an argument with her saying the max doses is 37.5mg which is one per day but she refused and got angry saying that I was the first pharmacist to call her about this. Regardless I refused to fill which throughly made her more upset which ultimately ended up with me hanging up on her. Ways to be 100% sure that a controlled substance prescription is legitimate?
I have 3 different scenarios: Patient A has been getting hydrocodone/apap 10-325mg #150, phentermine 37.5 mg #30, promethazine /codeine syrup 473 ml, and Soma 350mg #90 every single month, with a diagnosis of "chronic pain and emphysema" for the past 3+ years, verified by her pain-clinic doctor, who writes all four scripts for her every month. She doesn't seem to have any symptoms of emphysema as her lung capacity is quite good when she shouted, screamed and threatened us for an entire 8... Phentermine - no go for GA?
Just wanted peoples' opinions on this. I recently cancelled a case for an MRI in a claustrophobic patient with Stage 4 ESLD and CHF who was taking Phentermine as a diet drug. I've heard stories that it can lead to dysautonomia/intractable hypotension and I didn't want to risk it in an elective case in a sick patient. Anyone have experience dealing with this?
