Rectal gastric heterotopia with fundic gland polyps, mimicking a high-risk primary anorectal neoplasm: histological evidence of the effects of gastric acid-suppressing medication in a rectal ‘outlet patch’
Gastric heterotopia can occur in any part of the gastrointestinal tract but is rare in the rectum. This case of rectal gastric heterotopia is novel as the heterotopic mucosa contained fundic gland polyps, and there were histological changes in the gastric glands characteristic of proton pump inhibitor-related changes. These changes included apocrine-like luminal cytoplasmic protrusions in cystically dilated oxyntic  gland parietal cells. These are well described in the native stomachs of PPI treated patients, but this is the first report of these changes in heterotopic gastric mucosa to our knowledge. Inflammatory Causes of Dysphagia in Children
Gastroesophageal reflux (GER) and eosinophilic esophagitis (EoE) are the most common inflammatory causes of pediatric dysphagia, but several other less prevalent conditions should be considered. These conditions can affect one or several aspects of the swallowing process. In some inflammatory conditions dysphagia may be an early symptom. Esophagoscopy and instrumental swallow studies are often needed to determine the underlying diagnosis and best treatment plan. In some inflammatory conditions dysphagia can portend a worse outcome and need for more aggressive treatment of the underlying condition. Consultations with speech...

Focusing on mitochondria in the brain: from biology to therapeutics
AbstractMitochondria have multiple functions such as supplying energy, regulating the redox status, and producing proteins encoded by an independent genome. They are closely related to the physiology and pathology of many organs and tissues, among which the brain is particularly prominent. The brain demands 20% of the resting metabolic rate and holds highly active mitochondrial activities. Considerable research shows that mitochondria are closely related to brain function, while mitochondrial defects induce or exacerbate pathology in the brain. In this review, we provide comprehensive research advances of mitochondrial bio...

Diagnostic Approach to Interstitial Lung Diseases Associated with Connective Tissue Diseases
Semin Respir Crit Care Med DOI: 10.1055/s-0044-1785674Interstitial lung disorders are a group of respiratory diseases characterized by interstitial compartment infiltration, varying degrees of infiltration, and fibrosis, with or without small airway involvement. Although some are idiopathic (e.g., idiopathic pulmonary fibrosis, idiopathic interstitial pneumonias, and sarcoidosis), the great majority have an underlying etiology, such as systemic autoimmune rheumatic disease (SARD, also called Connective Tissue Diseases or CTD), inhalational exposure to organic matter, medications, and rarely, genetic disorders. This review ...

Anal and perianal disorders
Most anal diseases can be diagnosed by a careful history and examination. Management of haemorrhoids involves exclusion of more serious pathology, adequate explanation of the disorder, and dietary and defecatory advice; most do not require additional treatment. Outpatient procedures or surgical intervention can be required for more symptomatic cases. Anal fissures are initially managed with bulking laxatives and non-constipating analgesics; glyceryl trinitrate ointment is standard first-line treatment. (Source: Medicine)

Metastatic cutaneous squamous cell carcinoma accounts for nearly all squamous cell carcinomas of the parotid gland
AbstractPrimary squamous cell carcinoma of the parotid gland (pSCCP) has long been recognized as a separate entity and is included in the WHO classifications of salivary gland tumors. However, it is widely accepted among head and neck pathologists that pSCCP is exceptionally rare. Yet, there are many publications describing series of pSCCP and data from SEER and other cancer register databases indicate erroneously an increasing incidence of pSCCP. Importantly, pSCCP and metastatic (secondary) squamous cell carcinoma to the parotid gland (mSCCP) have nearly identical histological features, and the diagnosis of pSCCP should ...

Marchiafava-Bignami disease: why not Marchiafava-Bignami-Carducci disease?
AbstractThe Marchiafava-Bignami disease has a curious backstory, namely, the publication in 1898 of theContribution to the Study of Nonsuppurative Encephalitis (Carducci A in Riv Psicol Psichiat Neuropat 8-9:125-135,1898), in which the neo-graduate Agostino Carducci described the disease that the pathologists Ettore Marchiafava and Amico Bignami would report 5 years later. Molecular Regulation of NLRP3 Inflammasome Activation During Parasitic Infection
Biosci Rep. 2024 Apr 16:BSR20231918. doi: 10.1042/BSR20231918. Online ahead of print.ABSTRACTParasitic diseases are a serious global health concern, causing many common and severe infections, including Chagas disease, leishmaniasis, and schistosomiasis. The NLRP3 inflammasome belongs to the NLR (nucleotide-binding domain leucine-rich-repeat-containing proteins) family, which are cytosolic proteins playing key roles in the detection of pathogens. NLRP3 inflammasomes are activated in immune responses to Plasmodium,Leishmania, Toxoplasma gondii, Entamoeba histolytica,Trypanosoma cruzi and other parasites. The role of NLRP3 is ...

Poststroke Communication Ability Predicts Patient-Informant Discrepancies in Reported Activities and Participation
DISCUSSION: These findings highlight the importance of measuring participation by PROM. The PWA's experience of participation is not related to their level of language impairment and is predictably different from their care partners' perspective. As others have also reported, "mild" aphasia is not so mild to the PWA. Similarly, "severe" may not be so severe to the PWA. Further research is needed to connect these findings with counseling and caregiver education. Research on response processes (e.g., response shift) is also warranted.PMID:38625105 | DOI:10.1044/2024_AJSLP-23-00322 (Source: American Journal of Speech-Language Pathology)

Gesture's Role in the Communication of Adults With Different Types of Aphasia
CONCLUSIONS: Overall, our results provide evidence for a compensatory role of gesture in aphasia communication. Adults with Broca's aphasia, who showed the greatest speech production difficulties, also relied most on gesture, and this pattern was particularly pronounced in the third-person narrative context.PMID:38625101 | DOI:10.1044/2024_AJSLP-23-00046 (Source: American Journal of Speech-Language Pathology)
