DO Schools To Avoid/Go For?
Not applying this year but I only have had 1 MD interview from VCU (which has specific release dates for acceptances), on 6 holds (Drexel, George Washington, Penn State, West Virginia, UCSD (in-state for me), SUNY Upstate) and 5 MD rejections (4 out of state schools and Georgetown). The physician I'm shadowing is an MD but recommended DO Schools to apply to in case I don't get in this year (she's a family medicine doctor who also focuses on obesity medicine and prefers improving lifestyles... Bariatric surgery
Can a person who is not obese pay cash and get weight loss surgery? Or would the surgeon refuse it unless they are BMI 40 or some cutoff like that? (Source: Student Doctor Network)

sarcoma case drain site
I have a case of a pT1(4cm)N0MO Grade 2 sarcoma s/p resection by a good surg onc. Margins 4mm in two areas (deep and anterior), 1 mm "superficial" margin that surg onc feels is artifactually close (he took some skin). This was a superficial sarcoma in an obese leg. Just barely touched deep fascia but did not invade into muscle though surg onc took some muscle. Tumor involved saphenous vein and it had to be ligated. Patient already has some minor distal edema. I took a cartoon of a... Read more (Source: Student Doctor Network)

Outpatient tonsils question
The 2019 AAO-HNFS guidelines for tonsillectomy state that pediatric patients with severe OSA should be admitted for post-op monitoring and more specifically that obese patients with BMI >95% should undergo sleep studies to check for severe OSA. However, per their own admission, compliance with these guidelines is quite low, roughly in the 40% range. Psychiatrist treating obesity?
Given that some of the antiobesity medications (Contrave, stimulants, even phentermine-topiramate) as well as nonpharmacological options such as therapy (lifestyle modification, behavioral activation, motivational interviewing) are within the scope of practice for psychiatry, I was wondering how common it is for psychiatrists to be treating obesity outside of antipsychotic-induced weight gain? There are many more medications that we may not use (orlistat, GLP-1 agonists although this is... Read more (Source: Student Doctor Network)

How will you escape the pit (the ER)?
A few years into practice and already starting to wonder if EM is sustainable. I’ve considered plenty of other options. The ones I keep coming back to are wound care, obesity medicine, and maybe something like the VA or teaching if a med school will hire me. In the current world though, I’m not sure what the best exit strategy is. For those of you who plan to leave the pit, how are you getting out? (Source: Student Doctor Network)
