The Management of Nausea and Vomiting in Pregnancy and Hyperemesis Gravidarum (Green-top Guideline No. 69)
BJOG. 2024 Feb 4. doi: 10.1111/1471-0528.17739. Online ahead of print.ABSTRACTAn objective and validated index of nausea and vomiting such as the Pregnancy-Unique Quantification of Emesis (PUQE) and HyperEmesis Level Prediction (HELP) tools can be used to classify the severity of NVP and HG. [Grade C] Ketonuria is not an indicator of dehydration and should not be used to assess severity. [Grade A] There are safety and efficacy data for first line antiemetics such as anti (H1) histamines, phenothiazines and doxylamine/pyridoxine (Xonvea®) and they should be prescribed initially when required for NVP and HG (Appendix III). ...

Online ahead of print.ABSTRACTOgilvie syndrome is a functional disorder of colonic motility that causes acute and progressive dilation, which can lead to necrosis and perforation. Early diagnosis and management are essential to avoid serious complications. The case of a patient with Ogilvie syndrome refractory to medical and endoscopic treatment that required surgery is presented. This is a 68-year-old man with decreased level of consciousness and abdominal distension for 3 days. Last bowel movement 4 days ago. The data and tests appear in table 1. We are ...

Comparison of three different prophylactic treatments for postoperative nausea and vomiting after total joint arthroplasty under general anesthesia: a randomized clinical trial
CONCLUSION: Combined use of ondansetron, mosapride and three doses of dexamethasone can provide better antiemetic effectiveness, postoperative appetite, bowel function recovery, and pain relief than a single dose or ondansetron only.TRIAL REGISTRATION INFORMATION: The protocol was registered at the Chinese Clinical Trial Registry (ChiCTR1800015896, April 27, 2018).PMID:38291490 | PMC:PMC10826087 | DOI:10.1186/s40360-024-00735-9 (Source: BMC Pharmacology and Toxicology)

Experimental pharmacological approaches to reverse impaired awareness of hypoglycemia —a review
The colossal global burden of diabetes management is compounded by the serious complication of hypoglycemia. Protective physiologic hormonal and neurogenic counterregulatory responses to hypoglycemia are essential to preserve glucose homeostasis and avert serious morbidity. With recurrent exposure to hypoglycemic episodes over time, these counterregulatory responses to hypoglycemia can diminish, resulting in an impaired awareness of hypoglycemia (IAH). IAH is characterized by sudden neuroglycopenia rather than preceding cautionary autonomic symptoms. IAH increases the risk of subsequent sudden and severe hypoglycemic episo...

Treatment Options for Posttraumatic Headache: A Current Review of the Literature
AbstractPurpose of ReviewWe evaluate evidence-based treatments for posttraumatic headache (PTH), a secondary headache disorder resulting from traumatic brain injury (TBI), comprising nearly 4% of all symptomatic headache disorders. Utilizing recent publications, we aim to inform clinicians of current treatment methods.Recent FindingsThere is limited research on PTH treatment. A randomized controlled trial (RCT) of metoclopramide with diphenhydramine for acute PTH found that the treatment group (N  = 81) experienced more significant pain improvement than placebo by 1.4 points. For persistent PTH, an open-label study of ...

Comparison of the Effects of Govarcin Herbal Capsule and Metoclopramide for Alleviating Gastrointestinal Symptoms in Patients with Functional Dyspepsia: A Randomized Double-blind Clinical Trial
CONCLUSION: Govarcin herbal capsule can be used to remedy symptoms in patients with FD. It seems that Guarcin is as effective as metoclopramide in fighting symptoms of FD as no significant difference in efficacy has been demonstrated between them.PMID:38099534 | DOI:10.2174/0115748871266848231120112355 (Source: Reviews on Recent Clinical Trials)

Breastfeeding women in need of information about antiemetics for nausea and vomiting during pregnancy: a review of inquiries to a medicines information service
Conclusion: Based on our small review of spontaneous inquiries, there seems to be an information need about the use of antiemetics during lactation among women breastfeeding an older infant whilst suffering from NVP or HG. Addressing such use in guidelines for NVP and HG and/or other easily available information sources may be considered in order to balance out the restrictive information provided by the manufacturers. This could avoid potential unnecessary weaning of breastfeeding in an otherwise challenging situation. (Source: Frontiers in Pharmacology)

2108 Metoclopramide for analgesia in renal colic - a systematic review
Conclusion Evidence Synthesis: Two studies were included. Heterogeneity of primary outcome measurement and comparators rendered meta-analysis inappropriate; a narrative review is presented. Both studies showed evidence of analgesic effect; each demonstrated equivalence with alternative analgesic options (tenoxicam and Spasmofen), the latter being a combination analgesic no longer in routine use in clinical practice. Conclusions There is some evidence that metoclopramide may be an effective analgesic in the management of renal colic, with the highest quality study demonstrating analgesic properties similar to an intraveno...
