Clinical Profile, Treatment and Outcome of Thrombotic Thrombocytopenia Purpura (TTP) in Rituximab Era- an Experience from Tertiary Care Centre from North India
AbstractThrombotic thrombocytopenic purpura (TTP) is a microangiopathic hemolytic anemia (MAHA) resulting from severe deficiency of ADAMTS13. TTP is an acute medical emergency which requires early treatment with therapeutic plasma exchange. With the early use of Rituximab along with PLEX, early response is achieved and relapse rate has gone down. There is lack of published data from India regarding treatment and outcome of TTP. We retrospectively analyzed our data of 21 patients of TTP including 4 patients who had TA-TMA. TTP patients were treated with TPE, pulse methylprednisolone and rituximab. After a median follow up o...

Challenges in the treatment of pediatric Mycoplasma pneumoniae pneumonia
Conclusion:  This report summarizes the clinical significance of macrolide-resistant and refractory MP pneumonia and discusses the efficacy and safety of alternative drugs, with a stepwise approach to the management of MP pneumonia recommended from the viewpoint of clinical practice. What is Known:•Although MP pneumonia is usually a benign self-limited infection with response macrolides as first line therapy, severe life-threatening cases may develop if additional treatment strategies are not effectively implemented.•Macrolide-resistant and refractory MP pneumonia are two conditions that may complicate the clinical cour...

Extranodal Rosai-Dorfman disease manifesting as Sj ögren's syndrome combined with panuveitis and hypertrophic pachymeningitis: a case report and review of literature
We present the case of a 28-year-old woman who presented with recurrent blurred vision in her right eye for 3 months. She developed blindness and atrophy in her left eye a decade prior to presentation. She subsequently developed headache, fever, and impaired mental status. Cranial magnetic resonance imaging indicated hypertrophic pachymeningitis (HP), and 18F-fluoro-2-deoxy-2-d-glucose (FDG) positron emission tomography/computed tomography revealed significant FDG uptake in the left dura mater. Autoimmune testing revealed elevated anti-nuclear, anti-SS-A, and anti-SS-B antibody levels. Incisional biopsy of the atrophic eye...

Surgical management of intramedullary cervical spinal sarcoidosis complicated by transient unilateral weakness: A case report
CONCLUSION: Patients with neurosarcoidosis respond unpredictably to surgical decompression and require prolonged medical care, which is often unsuccessful.PMID:38628516 | PMC:PMC11021060 | DOI:10.25259/SNI_41_2024 (Source: Surgical Neurology International)

A Case of Adult-onset Still's Disease (AOSD) with Gastrointestinal Lesions
Intern Med. 2024 Apr 16. doi: 10.2169/internalmedicine.3412-23. Online ahead of print.ABSTRACTA 51-year-old woman with fever was admitted to our hospital. A computed tomography (CT) scan showed thickened colonic walls. Colonoscopy revealed erosion in the ileum and colon. Adult-onset Still's disease (AOSD) was diagnosed due to a subsequent sore throat and skin rash. Following AOSD treatment, methylprednisolone pulse therapy, followed by prednisolone and cyclosporine, was initiated. Despite achieving a temporary improvement, relapse occurred with fever, abdominal pain, with worsening CT and endoscopic findings. The reappeara...

GSE248553 Gene expression profile at single cell level of Gli1+ progenitors from the metaphysis of distal femur and proximal tibia in response to MPS.
Contributors : Puying Yang ; Fangyuan Shen ; Yu ShiSeries Type : Expression profiling by high throughput sequencingOrganism : Mus musculusGli1+ progenitors are considered as metaphyseal mesenchymal progenitors in the distal femur and proximal tibia under the growth plate. We used single cell RNA sequencing (scRNA-seq) to analyze the diversity of Gli1+ progenitors in response to methylprednisolone. (Source: GEO: Gene Expression Omnibus)

Efficacy and Safety of Methylprednisolone Pulse Therapy and Conventional Oral Prednisone for Pediatric Patients With Nephrotic Syndrome
CONCLUSIONS: High-dose methylprednisolone pulse therapy demonstrated better efficacy in treating pediatric NS patients, showing a shorter time to symptom relief, but it may also entail a higher risk of adverse reactions compared to conventional oral high-dose prednisone. Clinicians should consider the specific circumstances and needs of pediatric patients when selecting a treatment.PMID:38607215 (Source: Alternative Therapies in Health and Medicine)
