Preoperative considerations of new long-acting glucagon-like peptide-1 receptor agonists in diabetes mellitus
I've seen something about these drugs maybe replacing metformin as firstline treatment for DM2. Seems like this may become very common for us to see, and this is a nice review of the current concerns we may have: DEFINE_ME Full stomachs: "Nausea and vomiting are explained by direct central effects of GLP-1 and delayed gastric emptying. Both effects decrease over time with ongoing treatment because of tolerance and... Read more (Source: Student Doctor Network)

Fears of disliking certain aspects of medicine?
Does anyone else have this fear of only treating the symptoms but not the disease? If I have to spend the rest of my life prescribing Metformin for high blood sugar or statins for high cholesterol without attempting to fix the root cause, I’d rip my hair out. I wouldn’t take such a job even if it paid me 500k because I’d be treating symptoms rather than disease. In fact, I’d be much more satisfied with a 150k job where I could directly help a patient cut out processed sugar and excess... Read more (Source: Student Doctor Network)

Adding Metformin to VPA to control weight gain?
I have a moderately obese gentleman with Schizoaffective disorder and Meth use history who is still having a lot of AH and mood instability on VPA 1,000 mg, Risperdal 8 mg, Trazodone 150 mg and Wellbutrin 150 mg BID (has been snorting at group home a few times). His BMI is 37 and he wants to switch to Zyprexa as it worked for him in the past. I told him I would need labs before making any changes. But if I decide to cross-taper to Zyprexa, will adding the Metformin also help with the weight... Adding Metformin to VPA to control weight gain? (Source: Student Doctor Network)

Metformin and Metabolic Side Effects
I have not met one attending who prescribes metformin to combat metabolic side effects of our medications. 1. Why isn't it done more often? 2,. If you prescribe metformin, what dose and frequency do you prescribe? The articles I've read have varied from 500mg - 2500mg per day. (Source: Student Doctor Network)
