Venous Sinus Stenosis with Prominent Emissary Veins: A New Common Cranial MRI Finding of Mucopolysaccharidosis I [PEDIATRICS]
This study aims to describe previously unreported common cranial findings of sigmoid sinus stenosis with prominent emissary veins in MPSIH. A retrospective review was conducted of 66 patients with MPSIH who were treated at our institution. A total of 12 cranial MR imaging studies from 12 different patients demonstrating the venous sinus anatomy were reviewed. All 12 patients exhibited various degrees of sigmoid or transverse sinus stenosis. Eleven had various forms of emissary veins. Of those 12 patients with imaging of the venous sinuses, 9 had a lumbar puncture within the same months as the acquisition of the venogram wi...

Pharmacological interventions for the management of pain and discomfort during lumbar puncture in newborn infants
CONCLUSIONS: The evidence is very uncertain about the effect of topical anesthetics (lidocaine) compared to no anesthesia on successful lumbar puncture on first attempt, the number of attempts per lumbar puncture, episodes of bradycardia, episodes of desaturation, and occurrence of apnea. Compared to placebo, topical anesthetics (lidocaine or EMLA) may reduce pain assessed with the NFCS score. One ongoing study will assess the effects of systemic treatment.PMID:37767875 | DOI:10.1002/14651858.CD015594.pub2 (Source: Cochrane Database of Systematic Reviews)

Primary Cough Headache
AbstractPurpose of ReviewIn this review article, we summarize the most recent findings and observations in patients experiencing primary cough headache.Recent FindingsRecent studies draw attention to further characterization of patients with primary cough headache in a pulmonology clinic. There have also been recent case studies describing new therapy considerations, including noninvasive vagus nerve stimulation and lumbar punctures.SummaryPrimary cough headache has been a recognized entity since as early as 1932. Patients generally report a sudden onset, sharp headache lasting a few hours that is precipitated by Valsalva ...

Epstein –Barr virus reactivation‐related meningoencephalitis with transverse myelitis in pregnancy
Key Clinical MessageConsider the differential of Epstein –Barr virus (EBV) reactivation in pregnant women who develop progressive meningoencephalitis and transverse myelitis. EBV nucleic acid amplification should be considered in immunosuppressed patients.AbstractA 32-year-old G10P6M3K22 pregnant female presented to a regional hospital with progressive severe neurological and behavioral deficits. Magnetic resonance revealed cervical transverse myelitis. Lumbar puncture confirmed Epstein –Barr virus (EBV) DNA on a background of IgG-positive EBV serology. A diagnosis of EBV reactivation-related meningoencephalitis with t...

We would like to thank Drs. Gozdas and Dogan for reading our previous article and for providing us feedback on the treatment of neurosyphilis.1 Syphilis re-emergence cases have been reported and increased globally within the past 2 decades. Neurosyphilis accounts for 1.8%-3.5% of the cases and causes serious consequences.2 Diagnosis of neurosyphilis is confirmed via lumbar puncture spinal fluid test analyses —venereal disease research laboratory test, fluorescent treponemal antibody absorption test, and polymerase chain reaction. How to use lumbar puncture manometry in children
Measurement of cerebrospinal fluid pressure through lumbar puncture (LP) manometry is an essential practical skill all paediatricians should possess competency in. The ability to perform manometry is crucial in the diagnosis of idiopathic intracranial hypertension and can provide critical information on raised (or lowered) intracranial pressure in other clinical scenarios. Practitioners should be familiar with the procedure and in particular with equipment available to them locally. In this article, we will describe an approach to LP manometry. The online supplemental material includes an instructional video as well as sup...
