Methadone Rx
Hi everyone, There's a patient who has been filling methadone (~12 tabs/day= 120 mg) and klonopin at the pharmacy for almost 3 years. Upon review of the state's PDMP report, that's 369 mgEq/day. Per the dr's office the diagnosis code is M54.5 for chronic low back pain. I am relatively new to the pharmacy, and am not feeling comfortable filling the script. Under what circumstances, if any, would you fill the Rx? What if I don't fill it and the patient ends up going through withdrawal...? Any... Methadone Rx (Source: Student Doctor Network)

JAMA: Nocebo effect and Viscerosomatic Amplification...
The Potential Adverse Influence of Physicians’ Words Providing test results of dubious clinical significance also can lead to increased symptoms. For example, in a randomized study of acute low back pain, one group (n = 210) underwent spine imaging, whereas the other group (n = 211) did not. A treatment plan of conservative medical... Liklihood of RFA working after positive MBB
Just asking what you typically tell patients regarding efficacy or likelihood of improvement of RFL after they have had a positive MBB (or two). 80%? 90%? I personally have been doing just one MBB, most of patients are manual labor/industry guys with low back pain and have a hard time getting out of work for multiple appointments, and most insurers in my area only require 1 MBB. Just wondering if you have a certain speil regarding likelihood of RFA working in the lumbar spine after a + diag... Liklihood of RFA working after positive MBB (Source: Student Doctor Network)

chiro presentation on higher cost of PMR directed care
had a hospital meeting , there was a presentation on spine care, and presented by chiro, basically slide after slide indicating chiro treatment in the acute care phase for low back pain is superior, and PMR physician directed care is more expensive, I need some ammunition to refute that claim, I am just aware of Andy Haig's study that supports PMR getting involved early on with spine related pain, are there other studies or resources I can use to help me with a presentation. Anyone @ DO school refuse getting cervical adjustment in OMM?
Just curious as to those at DO programs can you refuse to have cervical adjustment performed on you? I interviewed at both MD and DO programs this cycle, and liked everything about the DO programs except the OMM aspect. Granted, lumbar manipulation has some support in literature for treatment in lower back pain--but other than that not so much. I ask this because of the proven link between cervical manipulation and vertebral artery dissection with stroke. Assuming most medical students... Stem Cell Therapy for Low Back Pain
Does anyone have an opinion on the injection of stem cells for the treatment of low back pain? I recently had an interested party approach me and ask if I was interested in performing this procedure for cash payments from patients ... it sounded good in theory, but I don't know ... SIS and other experts advocate for Oregon patients
SIS and 10 other medical societies have warned the Oregon Health Authority (OHA) that a cascade of unintended consequences can be expected from their significantly flawed coverage decision on corticosteroid injections for low back pain. The multi-society response highlights significant flaws in OHA’s coverage guidance methodology, includes citations on the effectiveness of injections, and warns of expected negative consequences of eliminating access to interventional spine procedures...
