Effect of Various Ancillary Operating Room Techniques on Wound Healing Outcomes After Total Hip Arthroplasty
CONCLUSION: Wound healing post THA is a complex process, influenced by various surgical techniques and intraoperative decisions. This systematic review meticulously examined three critical factors: the role of DVT prophylaxis, the implications of intraoperative drain usage, and the impact of different surgical approaches. Our analysis revealed that rivaroxaban and enoxaparin exhibit similar wound infection rates in THA. The decision to use intraoperative drains in the current literature indicates no definite advantage or disadvantage regarding wound problems with the use of closed-suction drainage in THA. In exploring surg...

Calibration-free electrochemical sensor to monitor factor-Xa inhibitors at the point-of-care anticoagulation therapy
This article presents a novel proof of concept for the blood plasma quantification of clinically relevant concentrations of direct oral anticoagulants, DOACs, including rivaroxaban and edoxaban, as well as low-molecular-weight heparins, LMWHs, such as enoxaparin and dalteparin, utilising a calibration-free disposable electrochemical sensor with co-facing electrodes. A dose-response curve was generated for rivaroxaban and edoxaban to demonstrate the sensor's ability to detect ≥9.00 ng mL-1 rivaroxaban and quantify it in the 11.0-140 ng mL-1 range. Similarly, the lower detection limit for edoxaban was 12.9 ng mL-1, with a ...

Multicenter study evaluating target attainment of anti ‐Factor Xa levels using various enoxaparin prophylactic dosing practices in adult trauma patients
This study compared BMI-, TBW-, EBV-based, and hybrid enoxaparin dosing strategies at achieving target prophylactic anti-Factor Xa (anti-Xa) levels in trauma patients. This was a multicenter, retrospective review of adult trauma patients who received enoxaparin twice daily for VTE prophylaxis and had at least one appropriately timed anti-Xa level (collected 3 to 6 hours after the previous dose after three consecutive doses) from January 2017 through December 2020. Patients were excluded if the hospital-specific dosing protocol was not followed or if they had thermal burns with>  20% body surface area involvement. The p...

Clinical outcomes associated with anti ‐Xa‐monitored enoxaparin for venous thromboembolism prophylaxis
ConclusionsThirty-day clinical outcomes in patients receiving enoxaparin for VTE prophylaxis were not improved by anti-Xa monitoring. Our results support current evidence-based guideline recommendations against anti-Xa monitoring for patients receiving enoxaparin for VTE prophylaxis. Factors associated with venous thromboembolic disease due to failed thromboprophylaxis
CONCLUSION: Our study suggests that the failed thromboprophylaxis is associated with high BMI, active cancer, leukocytosis, and ICU requirement. Future studies should evaluate whether there is benefit in adjusting the thromboprophylaxis scheme in patients with one or more of these factors.PMID:38057785 | DOI:10.1186/s12959-023-00566-4 (Source: Cancer Control)

Cannabis Arteritis and Recurrent Phlebitis
A previously healthy man in his late 30s presented with a 4-year history of ultrasound-confirmed recurrent upper and lower extremity superficial vein thrombosis (SVT) despite therapeutic apixaban, rivaroxaban, or enoxaparin (Figure  1). Although a lifelong nonuser of tobacco, he smoked marijuana daily for 20 years. With abstinence, the frequency of recurrent SVT resolved. Upon re-exposure, SVT recurred. Neither deep venous thrombosis nor pulmonary embolism was observed. (Source: Mayo Clinic Proceedings)

Event rates and risk factors for venous thromboembolism and major bleeding in a population of hospitalized adult patients with acute medical illness receiving enoxaparin thromboprophylaxis
CONCLUSION: There is substantial heterogeneity in the event rates for VTE and major bleeding in acute medically ill patients. History of VTE and cancer related hospitalization represent profiles with a high risk of VTE, where continued VTE prophylaxis may be warranted.PMID:38030465 | DOI:10.1016/j.ejim.2023.11.017 (Source: European Journal of Internal Medicine)
