Extreme photosensitivity and self-induced seizures with dramatic response to add-on Lorazepam in patient with WDR45 encephalopathy
Neurodegeneration with brain iron accumulation (NBIA) encompasses a range of inherited neurological disorders, each displaying a diverse array of symptoms. A specific subtype within this spectrum is known as beta-propeller protein-associated neurodegeneration (BPAN), which arises due to mutations in the WDR45 gene. This particular variant of NBIA presents with childhood developmental delay (DD), notably characterized by expressive speech delays and recurrent seizures. Of note BPAN is the sole X-linked form of NBIA, typically exhibiting a more severe course of disease or even fatality in males [1]. (Source: Seizure: Europea...

Predictors for and use of rescue medication in adults with epilepsy: A multicentre cross-sectional study from Germany
Despite adequate therapy with antiseizure medication (ASM), one-third of patients continue to have seizures, which may evolve into seizure emergencies, including seizure clusters, prolonged episodes, and status epilepticus, which represent life-threatening neurological emergencies with serious cerebral and systemic sequelae.[1,2] Rescue medications are given as needed to disrupt the progression of a seizure and forestall what would otherwise be a more prolonged or severe clinical event.[3,4]Benzodiazepines, such as diazepam, lorazepam, and midazolam, are positive allosteric modulators of GABAA receptors, whose activation l...

"Should We Phenobarb-it-All?" A Phenobarbital-Based Protocol for Non-Intensive Care Unit Trauma Patients at High Risk of or Experiencing Alcohol Withdrawal
The objective was to evaluate the effectiveness and safety of a phenobarbital-based protocol for the treatment of AWS in non-intensive care unit (ICU) trauma patients.METHODS: Adult non-ICU trauma patients at high risk of or experiencing AWS PRE and POST implementation of a phenobarbital-based protocol were included. Outcomes were AWS-related complications (AWS-RC), benzodiazepine use, adjunctive medication use, hospital length of stay (HLOS), and medication-related AEs. Subgroup analyses were performed on patients with traumatic brain injury (TBI), rib fractures, and at high risk of severe AWS.RESULTS: Overall, 110 patien...

Catatonia in patients with dementia: A descriptive study of clinical profiles and treatment response
CONCLUSION: The findings indicate a frequent co-occurrence of catatonia and dementia, highlighting treatability yet suggesting a potential for resistance to lorazepam, which varies by dementia diagnosis. Investigating the mechanisms underlying this resistance and the variability in treatment response is crucial for developing more precise therapeutic strategies.PMID:38564875 | DOI:10.1016/j.ajp.2024.104033 (Source: Asian Journal of Psychiatry)

Acute psychosis in a 35-year-old woman. Could it be related to a previous COVID-19 infection? A case report
Psychiatr Pol. 2023 Dec 31;57(6):1143-1149. doi: 10.12740/PP/152699. Epub 2023 Dec 31.ABSTRACTThe SARS-CoV-2 virus is the cause of the COVID-19 disease. Infection can take a wide variety of forms, from asymptomatic to severe, with numerous complications that can even lead to death. Since the beginning of the pandemic, numerous studies have been carried out to find out the exact expression of the virus. COVID-19 infection also increases the risk of developing neuropsychiatric symptoms, including psychosis. The paper presents the case of a 35-year-old woman with no prior psychiatric interview who developed acute psychosis af...

Development of an LC –MS/MS method for the determination of five psychoactive drugs in postmortem urine by optimization of enzymatic hydrolysis of glucuronide conjugates
ConclusionsWe have established an LC –MS/MS method for five psychoactive drugs in urine after enzymatic hydrolysis of glucuronide conjugates directly on extraction columns. The method was successfully applied to forensic autopsy samples. The established method will have broad applications, including forensic and clinical toxicologica l investigations. Incidence and Risk Factors for ICU-Associated Delirium in the Alert Geriatric Trauma Population
This study analyzed the overall incidence of delirium, comorbid conditions, injury patterns, and pharmacological risk factors for the development of delirium in an alert, geriatric trauma population.METHODS: IRB-approved, prospective, consecutive cohort series at two Southeastern Level 1 trauma centers from June 11 to August 15, 2023. Delirium was assessed using the Confusion Assessment Method (CAM) score. Comorbidities and medications were detailed from electronic medical records. Inclusion criteria: age ≥55, GCS ≥14, and ICU admission for trauma. Patients on a ventilator were excluded. Data was analyzed using SPSS ve...

Catatonia and melancholia interface: exploring a new paradigm for evaluation and treatment. A case series and literature review
ConclusionThe present study argues for a closer relationship between melancholia and catatonia based on our case series, historical review, overlap in phenomenology, and response to treatment. We propose provisional [Mahgoub] criteria for patients with severe depression and melancholia. The role of GABA agonists, such as lorazepam, can be explored as an option for patients with treatment-resistant depression who meet these criteria for melancholia.LimitationsAbsence of a standardized, systematic assessment tool and a small sample size.
