40M low ASCVD, will statin reduce risk of plaque over time?
Hi colleagues. I'm a new PCP. I have 40M with ASCVD risk < 2% who has LDL 150. His previous doctor put him in atorvastatin 10. The argument is that keeping the LDL down on a young guy will reduce the risk of plaque build up. Pt wants to know why I am waiting till he turns 60 for the ASCVD risk score to go up before starting statin. How do I explain the rationale to the patient? What Would You Do For This Diabetic With These ABPM Results?
A 76 years old  man, type 2 diabetic for 10 years, on 1.0 gram of Metformin, HBA1C of 8%, BMI of 28.5, No Microalbuminuria, small right carotid intima thickness but no plaques. He has a healthy Life Style with regular exercises and Diet, taking also 10 mg Atorvastatin, and on Amlodipine 5 mg/day had a 24 hour  Arterial Blood Pressure Monitoring requested by his GP because of his Hypertension. What Do You Think Of Pharmacogenomics to Restrict Statins Prescriptions?
In my humble opinion Statins is a Multi Billion Dollars Business, and are mostly prescribed for low risk patients with huge and huge NNTs for clinical benefits. Now, a recent Article suggest  Genotype based polymorphisms can be a good guide for Statins Prescriptions. As usual with any statin positive results it is being heavily advertised.
