Surgery associated with better survival for patients with advanced laryngeal cancer
Patients with advanced laryngeal cancer appear to have better survival if they are treated with surgery than nonsurgical chemoradiation. Approximately 11,000 to 13,000 cases of laryngeal cancer are diagnosed each year and squamous cell carcinoma accounts for the vast majority of these tumors. Prior to 1991, total surgical removal of the larynx with postoperative radiation was the standard of care for advanced cancer. Patients with oropharyngeal cancer report quality of voice, speech affected post-treatment
Oropharyngeal cancer patients treated with combined chemotherapy and radiation therapy reported a decrease in their voice and speech quality (VSQ) for up to one year after the completion of treatment, according to research. The study further shows that limiting the dose of radiation to the glottic larynx (GL) to less than 20 Gy resulted in a decrease in post-treatment VSQ problems, and that patient-reported VSQ indicated more adverse effects from treatment compared to independent physician assessment. EORTC head & neck cancer trial shows assessing HRQOL is valuable to both patients and their doctors
(European Organisation for Research and Treatment of Cancer) EORTC trial 24954 set out to compare two treatment schemes for patients with respectable hypopharyngeal and laryngeal cancers, and the results published in Cancer show that there is a trend towards worse HRQOL scores in patients receiving alternating chemoradiotherapy (alternating arm) as opposed to those given sequential induction chemotherapy and radiotherapy (sequential arm).
