Laparotomy in profesional Singer
This is happening right now. Im on call. We have a profesional singer whit a bad appy, 8 days of evolution, Healthy 27 year old. As im discussing the case with him he begs us not to intubate, as it will damage his singing ability. I discussed the case with my attending, she says we will try a T4 spinal hope for the best and if he does not tolerate we will intubate, my attending does not like lmas. What do you think? RVU production compensation questions
For any of the surgeons who have a contract that is wRVU based. I wanted to get some insight due to my own upcoming contraction negotiation (OBGYN). 1. If you are assisting a partner/another surgeon in a case, how are you as the assist getting compensated for this regarding the number of wRVU? Is modifier 80 applied. So if the surgery is an exploratory laparotomy (CPT 49000, wRVU 12.54) are you getting credit for 0.16 x 12.54=~2 wRVU? 2. Are your wRVUs being calculated on what your are... Advice from people who had needle based vasovagal syncope
I recently read in a study, 12% of med students had a vasovagal response in the operating theatre at one point or another. As I read on, it stated 0 of the 700 sample size reported a needle based phobia. The most commonly reported procedure that caused it was a laparotomy where 35% of them felt sick. Yet i watched one and felt nothing. So its safe to say some shenanigans is going on within my body cause I dont actually have a concious fear of needles or blood. Pretty sure Im just... what to do when you find ischemia of whole small intestine during laparotomy?
yesterday i operated on 57 years old male , had controlled DM , otherwise fit and healthy , presented with 4 days history of adbominal pain , vomit , low output. On examination and investigations found to have peritonities . A exploratory laparotomy was done under GA . Whole of his small bowel , from D-J flexure to almost 6 inch proximal to I-C junction was gangrenous.
