Improvement in right heart function following kidney transplantation in esrd patients: insights from speckle tracking echocardiography analysis
This study aimed to evaluate the effects of KTx on the right heart chambers in ESRD patients. 57 adult KTx candidates were enrolled in this prospective longitudinal study, while 49 of them were included in the final assessment. Patients underwent a comprehensive cardiac assessment, including conventional echocardiography, speckle tracking echocardiography, and three-dimensional heart modeling both before and after surgery. Echocardiographic assessments showed significant increases in right ventricular (RV) ejection fraction, RV fractional area change (RVFAC), tricuspid annular plain systolic excursion, RV fractional shorte...

Apparent Mineralocorticoid Excess in Israel: A Case Series and Literature Review
CONCLUSIONS: In this 11-year follow-up report of two Israeli families with AME, patients who presented early maintained long-term normal kidney function, while those who presented late progressed to ESKD. Nevertheless, despite early diagnosis and management, AME is commonly associated with serious complications of the disease or its treatment.PMID:38652803 | DOI:10.1093/ejendo/lvae049 (Source: European Journal of Endocrinology)

Mesenchymal stem cells promote ovarian reconstruction in mice
CONCLUSIONS: The presence of BM-MSCs in the artificial ovary was found to promote the survival of ovarian cells, as well as facilitate follicle formation and development. Since the cells didn't proliferate in the reconstituted ovary, this discovery suggests a potential new and safe method for the application of MSCs in clinical fertility preservation by enhancing the success rate of cryo-thawed ovarian tissues after transplantation.PMID:38650029 | DOI:10.1186/s13287-024-03718-z (Source: Cell Research)

Impaired kidney function: supporting the safe use of medicines for patients
This article provides an overview of the effects of impaired kidney function on pharmacokinetics and the importance of accurate drug dose adjustments for patients with related conditions. It also discusses various aspects of medicines use in patients with AKI, the pharmacological management of patients with CKD and the use of immunosuppressive therapy in patients who have had a kidney transplant. The authors consider the role of the nurse in ensuring safe medicines use for patients with impaired kidney function throughout the article.PMID:38646689 | DOI:10.7748/ns.2024.e12075 (Source: Nursing Standard)

Drug-drug interaction between tacrolimus and caspofungin in Chinese kidney transplant patients with different < em > CYP3A5 < /em > genotypes
CONCLUSION: Co-administration of caspofungin reduced tacrolimus blood levels and elevated the required daily dose of tacrolimus. In CYP3A5 non-expressers, co-administration of caspofungin had a significant effect on tacrolimus C0/D values. An approximate 10% increase in the weight-adjusted daily dose of tacrolimus in CYP3A5 non-expressers is recommended to ensure the safety of tacrolimus administration.PMID:38646424 | PMC:PMC11027596 | DOI:10.1177/20420986241243165 (Source: Adv Data)

Comparison of skin cancer risk between renal transplant recipients and patients with glomerular diseases in rural Queensland
CONCLUSION: In patients with a high risk of NMSC, reducing skin cancer risk should be considered in conjunction with the optimisation of treatment.PMID:38646861 | DOI:10.1111/ajr.13081 (Source: The Australian Journal of Rural Health)

From the Clinical to the Bench: Exploring the Insulin Modulation Effects of Tacrolimus and Belatacept
This study investigates whether the observed effects on PTD stem solely from CNI withdrawal or if belatacept influences PTD independently. The study assessed the impact of tacrolimus and belatacept on insulin secretion in MIN6 cells (a beta cell line) and rat islets. Tacrolimus and belatacept were administered to the cells and islets, followed by assessments of cell viability and insulin secretion. Tacrolimus impaired insulin secretion without affecting cell viability, while belatacept showed no detrimental effects on either parameter. These findings support clinical observations of improved HbA1c upon switching from tacro...

Disrupting B and T cell Collaboration in Autoimmune Disease: T cell engagers versus CAR T cell therapy?
Clin Exp Immunol. 2024 Apr 20:uxae031. doi: 10.1093/cei/uxae031. Online ahead of print.ABSTRACTB and T cells collaborate to drive autoimmune disease (AID). Historically, B and T cell (B-T cell) co-interaction was targeted through different pathways such as alemtuzumab, abatacept, and dapirolizumab with variable impact on B cell depletion (BCD), whereas the majority of patients with AID including rheumatoid arthritis, systemic lupus erythematosus, multiple sclerosis and organ transplantation benefit from targeted BCD with anti-CD20 monoclonal antibodies such as rituximab, ocrelizumab or ofatumumab. Refractory AID is a signi...

Extramedullary relapsed acute lymphoblastic leukemia presenting as asymptomatic acute kidney injury after hematopoietic stem cell transplantation
AbstractAcute kidney injury (AKI) is a common complication in children with hematological malignancies. Although AKI due to infiltration of tumor cells in children is rare, it negatively impacts treatment outcomes and increases the risk of mortality. We introduce a case of a child with acute lymphoblastic leukemia (ALL) who experienced kidney relapse resulting in asymptomatic AKI after remission from treatment, to remind clinicians not to overlook the primary disease in clinical judgment. In cases of unexplained AKI, kidney biopsy should be performed when feasible to get an accurate diagnosis and scientific treatment. In b...

Use of a donor kidney with known autosomal dominant polycystic kidney disease in a highly sensitised paediatric recipient
We report the use of an autosomal-dominant polycystic kidney disease (ADPKD) donor kidney in a paediatric recipient. A 14-year-old boy on haemodialysis for 4 years following loss of a first kidney transplant, highly sensitised, and with limited vascular options for ongoing dialysis access, was offered a deceased brain death donor transplant from a mid-30s donor with known ADPKD but normal kidney function and negligible proteinuria. After extensive discussion with the patient and family, discussing all alternative options and review of available literature, the kidney was accepted and implanted. Graft function was immediate...
