The role of HLA-B27 in the pathogenesis and diagnosis of axial spondyloarthritis : 50 years after discovery of the strong genetic association
This article gives an overview of the available knowledge on the topic.MATERIAL AND METHODS: This is a narrative review based on the experience of the authors.RESULTS: The HLA-B27 is a member of the HLA class I family of genes of the major histocompatibility complex (MHC). The prevalence of HLA-B27 in the central European population is approximately 8 %, i.e., the vast majority of carriers of HLA-B27+ remain healthy. The frequency of HLA-B27 shows a decline from north to south. The HLA-B27 explains only 30 % of the genetic burden of axSpA. The prevalence of the disease correlates with the frequency of HLA-B27 in the popula...

Why the regeneration of immunological tolerance by vaccination is difficult
Z Rheumatol. 2023 Dec 18. doi: 10.1007/s00393-023-01453-z. Online ahead of print.ABSTRACTAutoimmunity, including that involved in chronic inflammatory rheumatic diseases, seems to be the price we have to pay for our efficient immune system. It has the ability to precisely recognize pathogens and tumor cells, to efficiently fight them, to adapt to their alterations and provide specific immunity for a lifetime. "Inoculation", and more specifically "vaccination" takes advantage of this, either by transfer of protective antibodies (passive vaccination) or by using attenuated pathogens or parts of them by which a specific prote...

SARS-CoV-2 infection and autoimmunity
Z Rheumatol. 2023 Dec 18. doi: 10.1007/s00393-023-01455-x. Online ahead of print.ABSTRACTEven in the early phase of the corona pandemic in 2020, severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) was referred to as an "autoimmune virus". Since then, there have been numerous reports on the increased incidence of autoantibodies and autoimmune phenomena after SARS-CoV‑2 infections. On the one hand, autoantibodies can influence the course of the disease and on the other hand, they can lead to the first manifestation of new autoimmune diseases. In addition, a role of autoantibodies in the pathogenesis of post-c...
